L'occurrence de th\u00e9rapies inappropri\u00e9es \u00e9tait plus fr\u00e9quente pour les premi\u00e8res plateformes de d\u00e9fibrillateurs sous-cutan\u00e9s, mais a diminu\u00e9 de mani\u00e8re significative pour les dispositifs plus r\u00e9cents. Cette diminution a \u00e9t\u00e9 facilit\u00e9e par la mise en \u0153uvre syst\u00e9matique d'une proc\u00e9dure de d\u00e9pistage pr\u00e9implantatoire, la programmation de routine d'une zone conditionnelle et l'utilisation de nouveaux algorithmes pendant les phases de d\u00e9tection et de certification. La classification d'un cycle comporte trois phases successives : une phase de d\u00e9tection avec filtrage du signal d'entr\u00e9e, une phase de certification pour \u00e9liminer les signaux extracardiaques et une phase de d\u00e9cision.<\/p><ul><li>L'appareil utilise un seuil de d\u00e9tection adaptatif bas\u00e9 sur l'amplitude des deux cycles certifi\u00e9s pr\u00e9c\u00e9dents. Par cons\u00e9quent, la d\u00e9tection excessive des ondes P et\/ou T est favoris\u00e9e par la faible amplitude des complexes QRS. Lorsque l'onde R est de faible amplitude, l'appareil atteint rapidement des niveaux de sensibilit\u00e9 \u00e9lev\u00e9s, ce qui favorise la surd\u00e9tection de l'onde T, en particulier pendant l'exercice (diminution possible de l'amplitude de l'onde R et l\u00e9g\u00e8re augmentation de l'amplitude de l'onde T). Une onde R de faible amplitude est surtout observ\u00e9e chez les patients atteints de dysplasie ventriculaire droite arythmog\u00e8ne, de syndrome de Brugada, de sarco\u00efdose cardiaque ou de cardiomyopathie dilat\u00e9e.<\/li><li>Le niveau de sensibilit\u00e9 augmente parall\u00e8lement \u00e0 l'augmentation de la fr\u00e9quence cardiaque afin d'optimiser les capacit\u00e9s de d\u00e9tection en cas de tachycardie. En fonction de la fr\u00e9quence, la dur\u00e9e des p\u00e9riodes r\u00e9fractaires et l'ajustement du niveau de sensibilit\u00e9 d\u00e9pendent de l'amplitude des signaux pr\u00e9c\u00e9dents et de la programmation. Pour une m\u00eame limite inf\u00e9rieure de 200 bpm, la programmation de deux zones avec une zone de choc conditionnelle de 200 \u00e0 230 bpm rend l'appareil moins sensible et donc moins enclin \u00e0 la surd\u00e9tection que la programmation d'une seule zone de choc \u00e0 200 bpm.<\/li><li>Une fois le signal d\u00e9tect\u00e9, la phase de certification classe le cycle comme certifi\u00e9 ou comme bruit\/surd\u00e9tection. Les signaux dont la fr\u00e9quence est trop \u00e9lev\u00e9e (myopotentiels, interf\u00e9rences, rupture de sonde) sont consid\u00e9r\u00e9s comme du bruit et sont exclus du calcul de la fr\u00e9quence cardiaque. Les signaux passent ensuite par quatre \u00e9tapes de certification suppl\u00e9mentaires pour diagnostiquer le double comptage de l'onde R et\/ou la surd\u00e9tection de l'onde T.\u00a0<\/li><\/ul><p>\u00a0<\/p><p>Les diff\u00e9rents algorithmes permettant de diagnostiquer une surdensit\u00e9 de l'onde T, de l'onde P ou de l'onde R (double comptage) sont bas\u00e9s sur la mesure de l'amplitude minimale et maximale des complexes d\u00e9tect\u00e9s, sur la mesure des d\u00e9lais entre les d\u00e9tections et de la largeur des signaux, et sur la comparaison de la morphologie du signal d\u00e9tect\u00e9 avec la morphologie du complexe pr\u00e9c\u00e9dent et avec la morphologie du complexe de r\u00e9f\u00e9rence.<\/p><ul><li>Le premier algorithme compare la morphologie du signal d\u00e9tect\u00e9 avec la morphologie de r\u00e9f\u00e9rence ; une surdensit\u00e9 est suspect\u00e9e lorsqu'il y a une alternance entre deux signaux, l'un corr\u00e9l\u00e9 avec la r\u00e9f\u00e9rence et l'autre non corr\u00e9l\u00e9 : r\u00e9p\u00e9tition de s\u00e9quences signal corr\u00e9l\u00e9-signal non corr\u00e9l\u00e9-signal corr\u00e9l\u00e9, sugg\u00e9rant une surdensit\u00e9 d'un signal cardiaque surnum\u00e9raire.<\/li><li>Le second algorithme recherche un double comptage des complexes QRS en recherchant un sch\u00e9ma r\u00e9p\u00e9titif de morphologie altern\u00e9e dans les d\u00e9tections de couplage court et fixe.<\/li><li>Le troisi\u00e8me algorithme recherche l'alternance entre la dur\u00e9e de deux cycles (alternance entre deux intervalles) et n'est donc pas bas\u00e9 sur une analyse de la morphologie du signal.<\/li><li>Le quatri\u00e8me algorithme analyse la morphologie de trois complexes successifs d\u00e9tect\u00e9s et recherche la pr\u00e9sence d'une s\u00e9quence morphologie 1 (complexe QRS) - morphologie 2 (diff\u00e9rente de la premi\u00e8re ; onde T) - morphologie 1 (complexe QRS). Pour am\u00e9liorer les performances de l'algorithme, la largeur des complexes et des intervalles est \u00e9galement incluse dans l'analyse ; le fonctionnement de cet algorithme ne d\u00e9pend donc pas de la qualit\u00e9 de l'enregistrement morphologique de r\u00e9f\u00e9rence. Lorsque l'un des algorithmes diagnostique un surdosage, le signal est marqu\u00e9 comme non certifi\u00e9 par un - sur le trac\u00e9, sans pr\u00e9ciser quel algorithme a conduit au diagnostic.<\/li><\/ul><p>\u00a0<\/p><p>Ces diff\u00e9rents algorithmes peuvent \u00eatre tromp\u00e9s en cas de variabilit\u00e9 de l'amplitude ou de la morphologie du signal, de surdosage intermittent ou de double surdosage. En outre, certains algorithmes de diagnostic de surdosage n'ont \u00e9t\u00e9 int\u00e9gr\u00e9s que dans les plates-formes les plus r\u00e9centes et sont donc absents des appareils plus anciens. La surd\u00e9tection de l'onde T peut \u00eatre compliqu\u00e9e par l'apparition de multiples chocs inappropri\u00e9s avec un risque d'induction de la FV, car le choc peut \u00eatre d\u00e9livr\u00e9 au cours d'une p\u00e9riode vuln\u00e9rable.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-d5d2643 elementor-widget elementor-widget-heading\" data-id=\"d5d2643\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Surd\u00e9tection des myopotentiels<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-d6c7d06 elementor-widget__width-inherit elementor-widget elementor-widget-text-editor\" data-id=\"d6c7d06\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Les patients pr\u00e9sentant une faible amplitude du complexe QRS sont vuln\u00e9rables \u00e0 la surd\u00e9tection des myopotentiels. Le dispositif fonctionnera en permanence \u00e0 des niveaux de sensibilit\u00e9 \u00e9lev\u00e9s en raison des faibles amplitudes du complexe QRS, ce qui augmente le risque de surd\u00e9tection d'autres signaux intracardiaques ou extracardiaques de faible amplitude. Le grand espacement entre les \u00e9lectrodes de d\u00e9tection explique le risque accru de surd\u00e9tection des myopotentiels, qui peuvent avoir des origines diff\u00e9rentes selon le vecteur. Diverses man\u0153uvres peuvent \u00eatre utilis\u00e9es pour confirmer le diagnostic en reproduisant la surd\u00e9tection :<\/p><ul><li>vecteur primaire : les muscles impliqu\u00e9s peuvent \u00eatre 1) les muscles abdominaux (\u00e9lectrode proximale) ; le d\u00e9passement peut \u00eatre reproduit par une contre-man\u0153uvre, le patient \u00e9tant allong\u00e9 et essayant de soulever ses cuisses ; 2) le latissimus dorsi au niveau du compartiment ; le d\u00e9passement peut \u00eatre reproduit par une contre-man\u0153uvre, le patient tendant les bras devant lui et essayant de les abaisser ou de les soulever contre une r\u00e9sistance ; 3) le diaphragme ; une inspiration profonde peut reproduire le d\u00e9passement.<\/li><li>Vecteur secondaire : 1) le latissimus dorsi au niveau du compartiment ; 2) le grand pectoral (\u00e9lectrode distale) ; le d\u00e9passement peut \u00eatre reproduit par une man\u0153uvre de contrepoint, le patient joignant les mains devant lui et poussant vers l'int\u00e9rieur.<\/li><li>Vecteur suppl\u00e9mentaire : 1) le grand pectoral ; 2) les muscles abdominaux.\u00a0<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>","protected":false},"excerpt":{"rendered":"<p>Boston Scientific subcutaneous ICD &#8211; Oversensing Content Oversensing of P-waves and T-waves While most inappropriate therapies observed in patients with implanted intracardiac defibrillators are secondary to supraventricular rhythm disorders or lead dysfunction, T wave oversensing is the most common cause of inappropriate therapies for subcutaneous devices.
