L'enregistrement d'\u00e9pisodes de d\u00e9tection ventriculaire lorsqu'ils se produisent \u00e0 des fr\u00e9quences sinusales \u00e9lev\u00e9es doit faire suspecter une perte de stimulation pendant l'exercice. Il existe plusieurs causes possibles de perte de stimulation biventriculaire pendant l'exercice : sous-d\u00e9tection auriculaire, PVC fr\u00e9quents, arythmies auriculaires ou ventriculaires soutenues, raccourcissement de l'intervalle PR pendant l'exercice en dessous du d\u00e9lai AV programm\u00e9, vitesse de poursuite maximale programm\u00e9e trop faible par rapport \u00e0 la capacit\u00e9 du patient. La r\u00e9alisation d'un test d'effort permet de confirmer les conditions pr\u00e9alables au bon fonctionnement de l'appareil pendant l'exercice, \u00e0 savoir : une bonne d\u00e9tection auriculaire chez les patients sans incomp\u00e9tence chronotrope, une r\u00e9ponse adaptative appropri\u00e9e de la fr\u00e9quence chez les patients avec incomp\u00e9tence chronotrope, le fait que le point 2 :1 n'est pas atteint pendant l'exercice (d\u00e9lai AV raccourci et Auto PVARP), r\u00e9glage ad\u00e9quat de la vitesse maximale de poursuite et\/ou de la r\u00e9ponse adaptative maximale, absence d'arythmie pendant l'exercice (arythmie supraventriculaire, nombreux PVC, arythmie ventriculaire, tachycardie r\u00e9entrante du pacemaker).<\/p><p>Le trac\u00e9 enregistr\u00e9 chez ce patient montre qu'au cours d'une tachycardie sinusale d'effort, la fr\u00e9quence cardiaque du patient d\u00e9passe la fr\u00e9quence de suivi maximale programm\u00e9e. Chez un patient atteint d'un bloc auriculo-ventriculaire complet, cela se traduirait par un comportement de Wenckebach. Chez ce patient dont la conduction auriculo-ventriculaire est pr\u00e9serv\u00e9e, sans stimulation ventriculaire possible au-del\u00e0 de la fr\u00e9quence maximale de suivi, le d\u00e9lai AV est prolong\u00e9 et la conduction spontan\u00e9e r\u00e9appara\u00eet avec la perte de la stimulation biventriculaire au plus fort de l'effort. Une augmentation de la fr\u00e9quence maximale de suivi \u00e0 140 battements par minute a permis d'\u00e9liminer ce type d'\u00e9pisode. Cette nouvelle programmation a \u00e9t\u00e9 valid\u00e9e par la r\u00e9alisation d'un nouveau test d'effort qui a confirm\u00e9 la persistance de la stimulation et une capture biventriculaire au maximum des capacit\u00e9s du patient. Il n'y a pas de raison de freiner la fr\u00e9quence de suivi maximale programm\u00e9e en dessous des capacit\u00e9s d'acc\u00e9l\u00e9ration du patient pendant l'exercice. Ce type d'\u00e9pisode est cependant tr\u00e8s rarement symptomatique car il n'est pas associ\u00e9 \u00e0 une chute brutale de la fr\u00e9quence cardiaque.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-64c3587 elementor-widget elementor-widget-heading\" data-id=\"64c3587\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">2:1 bloc et comportement de wenckebach pendant l'exercice<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3594564 elementor-widget elementor-widget-text-editor\" data-id=\"3594564\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Les r\u00e9glages sp\u00e9cifiques des jeunes patients CRT avec un bloc auriculo-ventriculaire cong\u00e9nital pr\u00e9sentent certaines particularit\u00e9s \u00e0 conna\u00eetre. En effet, dans la grande majorit\u00e9 des cas, la conduction r\u00e9trograde, comme la conduction ant\u00e9rograde, est interrompue. Cette sp\u00e9cificit\u00e9 doit \u00eatre int\u00e9gr\u00e9e dans le processus de d\u00e9cision concernant la programmation optimale, le risque de PMT \u00e9tant nul. Un r\u00e9glage adapt\u00e9 doit donc permettre d'\u00e9viter la survenue du point 2:1 \u00e0 l'effort, souvent associ\u00e9 \u00e0 des sympt\u00f4mes tr\u00e8s invalidants. Chez ce patient, le point 2:1 a \u00e9t\u00e9 r\u00e9gl\u00e9 trop bas. La p\u00e9riode r\u00e9fractaire auriculaire totale, qui d\u00e9finit le point 2:1, correspond \u00e0 la somme du d\u00e9lai AV + PVARP.  Chez les patients jeunes, il est essentiel de repousser ce point 2:1 au-del\u00e0 des capacit\u00e9s d'acc\u00e9l\u00e9ration sinusale \u00e0 l'effort en programmant un d\u00e9lai AV adaptatif \u00e0 la cadence avec raccourcissement automatique \u00e0 l'effort et une PVARP la plus courte possible, la majorit\u00e9 de ces patients n'ayant pas besoin d'\u00eatre prot\u00e9g\u00e9s du risque de PMT qui peut \u00eatre consid\u00e9r\u00e9 comme nul. La programmation d'un d\u00e9lai AV minimum de 70 ms et d'un PVARP de 180 ms permet de repousser le point 2:1 bien au-del\u00e0 de 200 battements\/minute.\u00a0<\/p><p>Le r\u00e9glage doit \u00e9galement permettre la synchronisation de la stimulation biventriculaire jusqu'\u00e0 ce que le patient atteigne la vitesse maximale de suivi. Le comportement de Wenckebach est le plus souvent moins symptomatique que le comportement 2:1, bien qu'il doive \u00eatre \u00e9vit\u00e9 en r\u00e9glant une fr\u00e9quence de suivi maximale suffisamment \u00e9lev\u00e9e. Chez un patient CRT pr\u00e9sentant un bloc auriculo-ventriculaire complet et une fonction chronotrope pr\u00e9serv\u00e9e, lorsque le rythme sinusal s'acc\u00e9l\u00e8re et d\u00e9passe la vitesse de poursuite maximale programm\u00e9e, la stimulation ventriculaire \u00e0 la fin du d\u00e9lai AV programm\u00e9 serait associ\u00e9e \u00e0 un d\u00e9passement de la vitesse de poursuite maximale, ce qui est impossible. Le rythme ventriculaire ne peut plus suivre le rythme auriculaire en mode 1:1 et plafonne autour de cette valeur. Pour surmonter cette limitation, l'appareil prolonge le d\u00e9lai AV et un fonctionnement de Wenckebach de l'appareil se produit. Lorsque la fr\u00e9quence sinusale augmente au-del\u00e0 de la fr\u00e9quence de suivi maximale, la fr\u00e9quence de stimulation ventriculaire reste \u00e0 la valeur de suivi maximale et le d\u00e9lai AV d\u00e9tect\u00e9 observ\u00e9 est prolong\u00e9 \u00e0 chaque cycle de stimulation. Lorsqu'un \u00e9v\u00e9nement auriculaire d\u00e9tect\u00e9 se produit pendant le PVARP, il n'est pas synchronis\u00e9 avec la stimulation ventriculaire, ce qui entra\u00eene un battement manquant. L'onde P suivante tombe en dehors de la p\u00e9riode r\u00e9fractaire et d\u00e9clenche \u00e0 nouveau un d\u00e9lai AV programm\u00e9. Ce sch\u00e9ma se r\u00e9p\u00e8te tant que la fr\u00e9quence sinusale reste sup\u00e9rieure \u00e0 la fr\u00e9quence de suivi maximale programm\u00e9e. Les battements manquants sont moins fr\u00e9quents lorsque la fr\u00e9quence sinusale n'est que l\u00e9g\u00e8rement sup\u00e9rieure \u00e0 la fr\u00e9quence de suivi maximale et plus fr\u00e9quents lorsque la fr\u00e9quence sinusale d\u00e9passe largement la fr\u00e9quence de suivi maximale. Une fois que la fr\u00e9quence sinusale tombe en dessous de la fr\u00e9quence de suivi maximale, l'association AV 1:1 est r\u00e9tablie. Le comportement de Wenckebach peut \u00eatre d\u00e9fini par la vitesse \u00e0 laquelle le battement manquant se produit et par le rapport entre les \u00e9v\u00e9nements auriculaires d\u00e9tect\u00e9s et les \u00e9v\u00e9nements ventriculaires stimul\u00e9s (par exemple, 8\/7, 7\/6, 6\/5 ou 3\/2).
