corps du patient.<\/p><p><strong>Seuil d'activit\u00e9<\/strong><\/p><p>Comme l'activit\u00e9 d\u00e9tect\u00e9e varie d'un patient \u00e0 l'autre, la sensibilit\u00e9 au mouvement peut \u00eatre ajust\u00e9e en reprogrammant le param\u00e8tre du seuil d'activit\u00e9.<\/p><ul><li>en abaissant le seuil, le rythme de stimulation est modul\u00e9 par un mouvement moins important<\/li><li>une augmentation du seuil n\u00e9cessite des mouvements corporels plus marqu\u00e9s pour moduler la fr\u00e9quence de stimulation<\/li><li>si le seuil est augment\u00e9, les mouvements du corps doivent \u00eatre plus importants pour avoir un effet sur la fr\u00e9quence de stimulation<\/li><li>le compteur d'activit\u00e9 utilis\u00e9 pour calculer le taux du capteur est r\u00e9gul\u00e9 par le taux et l'amplitude du signal de l'acc\u00e9l\u00e9rom\u00e8tre<\/li><\/ul><p>\u00a0<\/p><p><strong>Taux programmables<\/strong><\/p><ul><li>le taux d'activit\u00e9s de la vie quotidienne (taux ADL) est le taux approximatif que le patient doit atteindre au cours d'un exercice mod\u00e9r\u00e9 (marche ou t\u00e2ches m\u00e9nag\u00e8res)<\/li><li>le taux sup\u00e9rieur du capteur est la limite sup\u00e9rieure du taux pilot\u00e9 par le capteur, atteinte lors d'un exercice vigoureux<\/li><li>un contr\u00f4le ind\u00e9pendant de la r\u00e9ponse au taux est assur\u00e9 dans la gamme des taux d'AVQ et dans la gamme des taux d'effort maximal<\/li><li>le stimulateur cardiaque maintient une relation lin\u00e9aire entre le signal du capteur d'activit\u00e9 et le taux de r\u00e9ponse<\/li><li>le syst\u00e8me d'optimisation des taux a un effet sur la pente de la relation lin\u00e9aire dans les deux gammes de taux<\/li><\/ul><p>\u00a0<\/p><p><strong>Points de consigne de la r\u00e9ponse tarifaire<\/strong><\/p><p>Les points de consigne d\u00e9finissent les deux pentes caract\u00e9ristiques d'une r\u00e9ponse de taux \u00e0 double pente.<\/p><ul><li>un point de consigne bas indique que des activit\u00e9s d'intensit\u00e9 relativement faible sont n\u00e9cessaires pour atteindre le taux maximum<\/li><li>le point de consigne ADL d\u00e9termine la vitesse de stimulation en fonction de la vitesse ADL<\/li><li>le point de consigne du taux sup\u00e9rieur (UR) d\u00e9termine le taux de stimulation vers la r\u00e9ponse au taux maximal<\/li><\/ul><p>\u00a0<\/p><p><strong>R\u00e9ponse automatique au taux<\/strong><\/p><p>Avec la r\u00e9ponse automatique au taux, l'optimisation de l'histogramme des taux continue \u00e0 ajuster la courbe des taux en faisant varier automatiquement les points de consigne.<\/p><ul><li>le taux de r\u00e9ponse est ajust\u00e9 sur la base de la programmation des param\u00e8tres de r\u00e9ponse ADL et de r\u00e9ponse \u00e0 l'effort<\/li><li>la r\u00e9ponse ADL contr\u00f4le la premi\u00e8re pente, qui d\u00e9termine l'intensit\u00e9 de l'augmentation de la fr\u00e9quence de stimulation entre la fr\u00e9quence inf\u00e9rieure et la fr\u00e9quence ADL<\/li><li>la r\u00e9ponse \u00e0 l'effort contr\u00f4le la deuxi\u00e8me pente, qui d\u00e9termine l'intensit\u00e9 de l'augmentation de la Ce profil de d\u00e9bit reste constant jusqu'\u00e0 ce que les param\u00e8tres soient reprogramm\u00e9s.<\/p><p>\u00a0<\/p><p><strong>Optimisation de l'histogramme des taux<\/strong><\/p><p>L'optimisation de l'histogramme des taux ajuste automatiquement la r\u00e9ponse du taux du patient entre les visites au cabinet.<\/p><ul><li>l'appareil recueille et enregistre les moyennes quotidiennes et, \u00e0 long terme, le pourcentage de temps pendant lequel la fr\u00e9quence indiqu\u00e9e par le capteur est \u00e0 diff\u00e9rentes fr\u00e9quences de stimulation<\/li><li>Il utilise ensuite les param\u00e8tres de r\u00e9ponse \u00e0 l'ADL et \u00e0 l'effort pour analyser le pourcentage de temps pendant lequel la fr\u00e9quence de stimulation reste dans la plage ADL par rapport \u00e0 la plage d'effort.<\/li><li>sur la base de comparaisons journali\u00e8res, le dispositif ajuste les points de consigne ADL, UR ou les deux<\/li><\/ul><p>\u00a0<\/p><p><strong>Adaptation du seuil d'activit\u00e9<\/strong><\/p><ul><li>lorsqu'un patient pr\u00e9sente une r\u00e9ponse insuffisante au cours de l'exercice, le seuil d'activit\u00e9 peut \u00eatre reprogramm\u00e9 \u00e0 un niveau inf\u00e9rieur (plus sensible) ; le niveau le plus sensible est \u201cLow\u201d (faible)\u201d<\/li><li>si la fr\u00e9quence de stimulation est \u00e9lev\u00e9e au repos, il peut \u00eatre n\u00e9cessaire de reprogrammer le seuil d'activit\u00e9 \u00e0 un niveau plus \u00e9lev\u00e9 (moins sensible) ; le niveau le moins sensible est \u201c\u00e9lev\u00e9\u201d.\u201d<\/li><\/ul><p>\u00a0<\/p><p><strong>Optimisation du profil tarifaire<\/strong><\/p><p>Avant de programmer les autres param\u00e8tres de r\u00e9ponse au d\u00e9bit, il faut d'abord v\u00e9rifier que les r\u00e9glages du d\u00e9bit inf\u00e9rieur, de l'ADL et du d\u00e9bit sup\u00e9rieur du capteur sont appropri\u00e9s pour chaque patient.<\/p><ul><li>si la reprogrammation des taux n'a pas optimis\u00e9 le profil de taux, il peut \u00eatre n\u00e9cessaire de reprogrammer les param\u00e8tres ADL et Exertion Response ; la reprogrammation de ces param\u00e8tres permet de concevoir un profil de taux qui correspond au style de vie individuel ou aux niveaux d'activit\u00e9 \u00e0 l'int\u00e9rieur de chaque fourchette de taux<\/li><li>la r\u00e9ponse ADL est ajust\u00e9e afin de d\u00e9finir la vitesse \u00e0 laquelle le patient atteint le taux ADL et la r\u00e9ponse Exertion pour la vitesse \u00e0 laquelle le patient atteint le taux d'effort<\/li><li>dans les deux cas, une valeur inf\u00e9rieure diminue, tandis qu'une valeur sup\u00e9rieure augmente le taux de r\u00e9ponse<\/li><\/ul><p>\u00a0<\/p><p><strong>R\u00e9glage manuel des points de consigne<\/strong><\/p><p>L'optimisation du profil de d\u00e9bit peut \u00eatre d\u00e9sactiv\u00e9e, ce qui permet de programmer manuellement les points de consigne.<\/p><ul><li>les points ADL et UR d\u00e9terminent le profil de la fr\u00e9quence de stimulation<\/li><li>le calcul des taux de r\u00e9ponse se poursuit comme pr\u00e9vu<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>","protected":false},"excerpt":{"rendered":"<p>Medtronic &#8211; Programming for exercise Content Rate-Adaptive pacing The activity sensor is an accelerometer embedded in the device, which detects the patient\u2019s body motion.
