Si la conduction AV est intacte, le dispositif reste en mode AAI. Le stimulateur passe en mode DDD car deux des quatre derniers intervalles A-A non r\u00e9fractaires ne pr\u00e9sentent pas d'\u00e9v\u00e9nement ventriculaire.<\/p><p>\u00a0<\/p><p><em>Reprise de la conduction AV<\/em><\/p><p>Apr\u00e8s avoir bascul\u00e9 en mode DDD, l'appareil v\u00e9rifie p\u00e9riodiquement la conduction AV afin de pouvoir revenir en mode AAI. Le premier contr\u00f4le de la conduction AV a lieu 1 minute apr\u00e8s le passage en mode DDD. Pendant le contr\u00f4le de la conduction, l'appareil passe en mode de stimulation AAI pendant un cycle.<\/p><p>Si l'intervalle A-A suivant comprend un battement ventriculaire d\u00e9tect\u00e9, la v\u00e9rification de la conduction r\u00e9ussit. Le temps entre les contr\u00f4les de conduction double alors (2, 4, 8 ... minutes, jusqu'\u00e0 un maximum de 16 heures) \u00e0 chaque \u00e9chec du contr\u00f4le de conduction.<\/p><p><strong>Sp\u00e9cificit\u00e9s de l'algorithme<\/strong><\/p><p><em>P\u00e9riode r\u00e9fractaire auriculaire<\/em>Lorsque le MVP fonctionne en mode AAI, la p\u00e9riode r\u00e9fractaire auriculaire n'est pas programmable. Elle est automatiquement ajust\u00e9e en fonction de la fr\u00e9quence cardiaque actuelle : 600 ms pour les fr\u00e9quences inf\u00e9rieures \u00e0 75 bpm et 75% de l'intervalle ventriculaire pour les fr\u00e9quences \u00e9gales ou sup\u00e9rieures \u00e0 75 bpm. Une activit\u00e9 auriculaire d\u00e9tect\u00e9e au cours de cette p\u00e9riode r\u00e9fractaire auriculaire dynamique ne r\u00e9initialise pas l'intervalle d'\u00e9chappement A-A afin d'\u00e9viter des passages inappropri\u00e9s au DDD en pr\u00e9sence de contractions auriculaires pr\u00e9matur\u00e9es non conduites.<\/p><p><em>\u00a0PVC et tachyarythmies ventriculaires<\/em>Le MVP fonctionne en mode AAI et inhibe la stimulation auriculaire en r\u00e9ponse aux PVC, aux s\u00e9ries de PVC et aux \u00e9pisodes de tachyarythmie ventriculaire, car la suppression de la chambre transversale peut affecter la d\u00e9tection des arythmies ventriculaires. Ce comportement a pour but d'\u00e9viter une stimulation auriculaire inutile lorsque la fr\u00e9quence ventriculaire est plus rapide que la fr\u00e9quence de stimulation. Il permet \u00e9galement aux fonctions de d\u00e9tection des tachyarythmies de fonctionner sans \u00eatre perturb\u00e9es par les p\u00e9riodes de suppression caus\u00e9es par la stimulation auriculaire.<\/p><p><em>Pause ventriculaire et limite inf\u00e9rieure de la fr\u00e9quence<\/em>Le mode MVP permet, en cas de perte brutale de la conduction auriculo-ventriculaire, avant le passage en mode DDD, des pauses occasionnelles pouvant aller jusqu'\u00e0 deux fois l'intervalle de fr\u00e9quence inf\u00e9rieure programm\u00e9. Il est donc recommand\u00e9 de programmer la fr\u00e9quence inf\u00e9rieure \u00e0 50 bpm ou plus pour r\u00e9duire la dur\u00e9e de la pause en cas de perte de la conduction auriculo-ventriculaire.<\/p><p><em>\u00a0Bloc cardiaque complet<\/em>Pour les patients pr\u00e9sentant un bloc AV complet, l'appareil fonctionne en mode DDD de mani\u00e8re persistante. Toutes les 16 heures, l'appareil v\u00e9rifie la conduction AV, ce qui se traduit par un seul battement ventriculaire interrompu. Si cette pause est symptomatique, la programmation du mode DDD peut \u00eatre plus appropri\u00e9e.<\/p><p><em>Intervalles PR longs<\/em>L'appareil reste en mode AAI : pour les patients ayant un long intervalle PR, l'appareil reste en mode AAI. Les modes permanents DDDR ou DDD peuvent \u00eatre plus appropri\u00e9s pour les patients pr\u00e9sentant un bloc AV du premier degr\u00e9 symptomatique, en particulier s'il n'y a pas de r\u00e9duction de l'intervalle PR pendant l'exercice.<\/p><p><em>\u00a0Op\u00e9ration imm\u00e9diatement apr\u00e8s l'implantation<\/em>L'appareil est livr\u00e9 en mode MVP et fonctionne initialement en mode DDD. Dans la premi\u00e8re version de l'algorithme MVP, l'appareil ne basculait pas en cas d'intervalles PR longs. \u00c0 la demande de certains m\u00e9decins confront\u00e9s \u00e0 des patients restant symptomatiques en cas de PR long \u00e0 l'effort, l'algorithme a \u00e9t\u00e9 modifi\u00e9 en cons\u00e9quence.<\/p><p>L'appareil surveille l'intervalle PR (AS-VS) et AR (AP-VS). Le changement le plus important de la version actualis\u00e9e est que l'appareil n'attend pas l'\u00e9v\u00e9nement auriculaire suivant pour d\u00e9tecter la perte de conduction AV. La dur\u00e9e de l'intervalle AA d\u00e9pend des mesures pr\u00e9c\u00e9dentes des intervalles PR (420 ms si PR  320 ms, l'intervalle AA sera allong\u00e9 de 100 ms jusqu'\u00e0 une limite de 600 ms (par exemple, si l'intervalle PR moyen est de 400 ms, l'intervalle AA sera de 500 ms). Le timing est le m\u00eame, que les s\u00e9quences AV pr\u00e9c\u00e9dentes aient \u00e9t\u00e9 AP-VS ou AS-VS. La stimulation ventriculaire est ensuite d\u00e9livr\u00e9e 80 ms apr\u00e8s la stimulation auriculaire.<\/p><p>L'objectif de cette s\u00e9quence (stimulation auriculaire avant la stimulation ventriculaire) est d'\u00e9viter l'apparition d'une pause tout en maintenant la synchronisation auriculo-ventriculaire afin d'emp\u00eacher la conduction r\u00e9trograde et de r\u00e9duire ainsi le risque de tachycardie induite par le stimulateur cardiaque. Le d\u00e9lai AA minimal de 420 ms permet \u00e9galement d'\u00e9viter la stimulation au cours d'une p\u00e9riode de vuln\u00e9rabilit\u00e9 auriculaire et d'\u00e9viter l'induction d'arythmies auriculaires.<\/p><p>\u00a0<\/p><p><strong>R\u00e9duction de la pause ventriculaire maximale apr\u00e8s le PVC :\u00a0<\/strong>La synchronisation VA est ajust\u00e9e (sur la base de la synchronisation des \u00e9v\u00e9nements pr\u00e9c\u00e9dents) pour obtenir un intervalle VV minimum de 1000 ms lorsqu'une PVC se produit. L'historique des intervalles AV est utilis\u00e9 pour d\u00e9terminer l'intervalle VA afin de r\u00e9duire la pause apr\u00e8s les PVC. Si la v\u00e9rification de la conduction est r\u00e9ussie, mais que l'appareil repasse en mode DDD(R) dans les 15 secondes, la v\u00e9rification de la conduction est \u00e0 nouveau consid\u00e9r\u00e9e comme un \u00e9chec et l'intervalle de v\u00e9rification de la conduction n'est pas r\u00e9initialis\u00e9 \u00e0 1 minute, mais reste \u00e0 son niveau actuel.<\/p><p><strong>Retarder la v\u00e9rification de la conduction \u00e0 des taux \u00e9lev\u00e9s :\u00a0<\/strong>pour \u00e9viter l'apparition d'une onde P bloqu\u00e9e pendant l'exercice, si la fr\u00e9quence du capteur est sup\u00e9rieure \u00e0 90 bpm et que l'intervalle AR moyen est sup\u00e9rieur \u00e0 300 ms, la v\u00e9rification de la conduction est retard\u00e9e d'une minute. Si les conditions sont toujours r\u00e9unies apr\u00e8s 1 minute, la v\u00e9rification de la conduction est \u00e0 nouveau retard\u00e9e d'une minute.<\/p><p><strong>\u00a0Crossblanking :\u00a0<\/strong>la suppression ventriculaire suivant la stimulation auriculaire a \u00e9t\u00e9 r\u00e9duite de 30 ms \u00e0 10 ms pour r\u00e9duire le risque de sous-d\u00e9tection d'une PVC tardive.<\/p><p><strong>Le mode MVP passe en mode diagnostic :\u00a0<\/strong>le journal de diagnostic a \u00e9t\u00e9 ajout\u00e9 pour comprendre la nature et la fr\u00e9quence des changements de mode. L'appareil conserve les 10 derniers changements de mode MVP vers le mode DDD et le nombre de changements de mode MVP depuis la derni\u00e8re session.
