LV1, prox LV2, prox LV3 et prox LV4, dist LV1 \u00e9tant l'\u00e9lectrode la plus distale, et 17 configurations de programmation diff\u00e9rentes sont disponibles.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-cda730b elementor-widget elementor-widget-heading\" data-id=\"cda730b\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h4 class=\"elementor-heading-title elementor-size-default\">Seuil automatique du ventricule gauche<\/h4>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-35e5292 elementor-widget elementor-widget-text-editor\" data-id=\"35e5292\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>The PaceSafe LV function is available for all unipolar and bipolar stimulation configurations, though is not available with the IS4 (quadripolar lead) defibrillators.<\/p><p>Dans le cadre ambulatoire, le seuil est automatiquement mesur\u00e9 toutes les 21 heures avec les param\u00e8tres programm\u00e9s suivants : d\u00e9lai AV rythm\u00e9 \u00e0 140 ms ; d\u00e9lai AV d\u00e9tect\u00e9 \u00e0 110 ms ; la stimulation RV de secours se produit tout au long du test LV avec une pr\u00e9excitation LV de 80 ms. L'amplitude initiale correspond \u00e0 l'amplitude maximale programmable, diminuant par paliers de 0,5 V au-dessus de 3,5 V et par paliers de 0,1 V en dessous de 3,5 V.<\/p><p>La mesure du seuil est bas\u00e9e sur une analyse de la r\u00e9ponse \u00e9voqu\u00e9e. Le canal de r\u00e9ponse \u00e9voqu\u00e9e est un canal de d\u00e9tection unipolaire qui utilise l'\u00e9lectrode LV distale ou proximale, selon la configuration de stimulation programm\u00e9e. En cas d'\u00e9chec du test quotidien, la fonction LV PaceSafe revient \u00e0 la sortie mesur\u00e9e pr\u00e9c\u00e9demment, et jusqu'\u00e0 trois nouvelles tentatives sont alors effectu\u00e9es, \u00e0 intervalles d'une heure. Si le test quotidien \u00e9choue pendant 4 jours cons\u00e9cutifs, une alerte est d\u00e9clench\u00e9e et la fonction LV PaceSafe passe en mode \u201c\u00c9chec\u201d (sortie d\u00e9livr\u00e9e = 5 V\/0,4 ms).<\/p><p>Si l'adaptation automatique a \u00e9t\u00e9 programm\u00e9e, suite aux mesures effectu\u00e9es \u00e0 21 heures d'intervalle, la sortie est alors adapt\u00e9e avec une marge de s\u00e9curit\u00e9 programmable entre 0,5 et 2,5 V, sans v\u00e9rification cycle par cycle de la capture. Au lieu d'\u00eatre adapt\u00e9e relativement \u00e0 la toute derni\u00e8re mesure, la sortie de stimulation LV est r\u00e9gl\u00e9e avec une marge de s\u00e9curit\u00e9 programmable ajout\u00e9e au seuil mesur\u00e9 lors des 7 derniers tests r\u00e9ussis effectu\u00e9s pendant le suivi ambulatoire. Cela permet d'augmenter rapidement l'amplitude de la stimulation en cas d'augmentation soudaine du seuil.
