De nos jours, les \u00e9checs de d\u00e9fibrillation sont rares, la majorit\u00e9 des patients sont implant\u00e9s en pr\u00e9vention primaire, et l'ATP peut mettre fin \u00e0 une grande partie des TV rapides. En outre, la puissance maximale des chocs d\u00e9livr\u00e9s par les derniers d\u00e9fibrillateurs implantables est plus \u00e9lev\u00e9e et associ\u00e9e \u00e0 une plus grande marge de s\u00e9curit\u00e9. Cela a incit\u00e9 de nombreux centres d'implantation \u00e0 reconsid\u00e9rer la n\u00e9cessit\u00e9 d'un test syst\u00e9matique \u00e0 la fin de la proc\u00e9dure et, au lieu de cela, \u00e0 proc\u00e9der au cas par cas.<\/p><p>L'induction de la FV permet de tester le bon fonctionnement du dispositif implant\u00e9 et de v\u00e9rifier :<\/p><ul><li>l'int\u00e9grit\u00e9 du syst\u00e8me de haute tension\u00a0<\/li><li>la d\u00e9tection fiable et la d\u00e9tection r\u00e9p\u00e9t\u00e9e des signaux endocardiques pendant la FV \u00a0<\/li><li>une marge de s\u00e9curit\u00e9 suffisante pour la d\u00e9fibrillation.<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7df43d1 elementor-widget elementor-widget-heading\" data-id=\"7df43d1\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Induction de la fibrillation ventriculaire<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f3c0bd7 elementor-widget elementor-widget-text-editor\" data-id=\"f3c0bd7\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Quatre moyens diff\u00e9rents sont disponibles pour induire la FV dans les appareils Boston Science : choc sur T, HF Burst et induction de la FVib.<\/p><p>1. Choc sur T : Une arythmie ventriculaire (le plus souvent une FV) est provoqu\u00e9e par l'administration d'un choc de faible \u00e9nergie pendant la p\u00e9riode de vuln\u00e9rabilit\u00e9 ventriculaire. Induction de la fibrillation ventriculaire : stimulation du ventricule droit \u00e0 travers les \u00e9lectrodes \u00e0 haute tension \u00e0 un rythme et une amplitude tr\u00e8s \u00e9lev\u00e9s, bien que l'intensit\u00e9 de l'impulsion soit programmable d'une forme d'onde faible de 9 V\/1,1 ms \u00e0 une longueur de cycle de 50 ms (20 Hz) \u00e0 une forme d'onde forte de 15 V\/1,1 ms \u00e0 une longueur de cycle de 50 ms. Le train d'impulsions est d\u00e9livr\u00e9 tant que l'op\u00e9rateur appuie sur l'interrupteur et que la communication t\u00e9l\u00e9m\u00e9trique est maintenue (jusqu'\u00e0 15 secondes).
