Par cons\u00e9quent, la programmation de la sensibilit\u00e9 doit \u00eatre \u00e9lev\u00e9e et les p\u00e9riodes r\u00e9fractaires doivent \u00eatre courtes. Contrairement \u00e0 un stimulateur cardiaque, le seuil de d\u00e9tection n'est pas programm\u00e9 \u00e0 une valeur fixe, mais s'adapte automatiquement en fonction de l'amplitude de l'onde R pr\u00e9c\u00e9dente. La sensibilit\u00e9 augmente ensuite tout au long du cycle, \u00e0 la recherche d'un \u00e9ventuel signal de faible amplitude.<\/p><p>Une fois qu'un signal a \u00e9t\u00e9 d\u00e9tect\u00e9, \u00e0 la fin de la p\u00e9riode r\u00e9fractaire, le niveau d'adaptation de la sensibilit\u00e9 commence \u00e0 un pourcentage programmable de l'amplitude de l'onde R d\u00e9tect\u00e9e, reste fixe pendant un d\u00e9lai d'adaptation programmable, avant d'augmenter progressivement jusqu'au niveau de sensibilit\u00e9 programm\u00e9.\u00a0<\/p><p>Pour obtenir une d\u00e9tection pr\u00e9cise de la vraie FV sans surd\u00e9tecter l'onde T pendant le rythme sinusal, il est possible de modifier les diff\u00e9rents r\u00e9glages des param\u00e8tres suivants :<\/p><ul><li>la p\u00e9riode r\u00e9fractaire ventriculaire d\u00e9tect\u00e9e ; il ne s'agit pas d'une v\u00e9ritable p\u00e9riode de suppression puisque l'appareil recherche l'amplitude maximale du signal apr\u00e8s que le seuil de d\u00e9tection a \u00e9t\u00e9 atteint ; cette p\u00e9riode r\u00e9fractaire doit \u00eatre courte (2 valeurs programmables, 125 ou 157 ms) pour \u00e9viter la d\u00e9tection multiple d'un m\u00eame signal de d\u00e9polarisation, tout en pr\u00e9servant la capacit\u00e9 de d\u00e9tecter une tachycardie tr\u00e8s rapide.<\/li><li>le d\u00e9but du seuil : \u00e0 l'expiration de la p\u00e9riode r\u00e9fractaire d\u00e9tect\u00e9e, le seuil de d\u00e9tection s'ajuste automatiquement \u00e0 un pourcentage de l'amplitude maximale du signal d\u00e9tect\u00e9 avec une valeur maximale absolue de 6 mV dans le ventricule et une valeur minimale de 1 mV. Le d\u00e9but du seuil apr\u00e8s d\u00e9tection peut \u00eatre fix\u00e9 \u00e0 50, 62,5, 75 ou 100%. Si l'amplitude du signal ventriculaire est &gt; 6 mV, le seuil de d\u00e9part est de 6 mV si 100% ou de 3 mV si 50%. Si l'amplitude du signal est &lt; 1 mV, le seuil de d\u00e9part est de 1 mV.<\/li><li>le d\u00e9lai de d\u00e9croissance : ce param\u00e8tre d\u00e9termine le temps pendant lequel, apr\u00e8s la p\u00e9riode r\u00e9fractaire d\u00e9tect\u00e9e, le seuil reste au niveau programm\u00e9 avant d'entamer sa d\u00e9croissance jusqu'au niveau de sensibilit\u00e9 programm\u00e9. Les d\u00e9fibrillateurs les plus modernes permettent de programmer plusieurs zones de d\u00e9tection des arythmies en fonction des intervalles RR d\u00e9tect\u00e9s. Chaque zone poss\u00e8de ses propres crit\u00e8res de fr\u00e9quence et de dur\u00e9e et peut \u00eatre programm\u00e9e sp\u00e9cifiquement en vue d'identifier et de traiter les arythmies.  Le param\u00e8tre Intervalle de d\u00e9tection\/Taux est l'intervalle\/le taux qui doit \u00eatre d\u00e9pass\u00e9 dans chaque zone de taux pour \u00eatre pris en compte dans la d\u00e9tection d'une tachyarythmie. Des taux plus \u00e9lev\u00e9s (en bpm) signifient des longueurs de cycle plus courtes (en millisecondes).  L'intervalle\/taux de d\u00e9tection est programmable ind\u00e9pendamment dans chaque zone de fr\u00e9quence. Chaque intervalle de d\u00e9tection doit \u00eatre au moins 30 ms plus long que l'intervalle de d\u00e9tection le plus rapide suivant.\u00a0<\/p><p>Une \u00e0 trois zones de d\u00e9tection peuvent \u00eatre configur\u00e9es \u00e0 la discr\u00e9tion du m\u00e9decin. Les zones permettent au clinicien d'administrer diff\u00e9rentes th\u00e9rapies \u00e0 des troubles du rythme de fr\u00e9quences diff\u00e9rentes. En outre, il est possible de configurer une \u201czone de surveillance\u201d dans laquelle le DAI continue d'\u00e9valuer le rythme (les diagnostics sont recueillis et les discriminateurs peuvent \u00eatre appliqu\u00e9s) mais ne d\u00e9livre pas de traitement.<\/p><p><img decoding=\"async\" src=\"https:\/\/cardiocases.com\/sites\/default\/files\/detection_6.jpg\" alt=\"\" \/>Le nombre de zones et l'\u00e9ventail des taux programm\u00e9s doivent \u00eatre adapt\u00e9s aux caract\u00e9ristiques du patient, \u00e0 ses ant\u00e9c\u00e9dents et aux indications de l'implant.\u00a0<\/p><p><strong>Pr\u00e9vention secondaire :<\/strong><\/p><p>Chez un patient souffrant d'insuffisance cardiaque, des zones de fr\u00e9quence cardiaque sont programm\u00e9es en fonction du cycle de tachycardie provoquant l'implantation de l'appareil. La limite inf\u00e9rieure programm\u00e9e de la zone de tachycardie est g\u00e9n\u00e9ralement inf\u00e9rieure de 20 bpm \u00e0 la fr\u00e9quence de la tachycardie clinique, car l'introduction ou l'ajout d'un traitement antiarythmique apr\u00e8s l'implantation du d\u00e9fibrillateur pourrait ralentir la fr\u00e9quence de la tachycardie.<\/p><p><strong>Pr\u00e9vention primaire :\u00a0<\/strong><\/p><p>Chez les patients souffrant d'insuffisance cardiaque, le risque de d\u00e9velopper une TV apr\u00e8s l'implantation du dispositif est \u00e9lev\u00e9, ce qui justifie probablement la programmation de deux zones de d\u00e9tection et de traitement de la TV en plus de la zone de FV. Le choix de deux zones distinctes de d\u00e9tection de la TV pr\u00e9sente l'avantage de permettre le traitement sp\u00e9cifique des tachycardies tr\u00e8s rapides, en favorisant la stimulation anti-tachycardique indolore plut\u00f4t que l'administration de chocs \u00e9lectriques. Ceci a \u00e9t\u00e9 confirm\u00e9 dans les \u00e9tudes cliniques PAIN FREE I et II, o\u00f9 la stimulation anti-tachycardique a permis d'\u00e9viter 75% des chocs dans la zone de tachycardie rapide, am\u00e9liorant de mani\u00e8re significative la qualit\u00e9 de vie sans augmenter de mani\u00e8re significative le taux d'\u00e9v\u00e9nements ind\u00e9sirables.\u00a0<\/p><p><strong>Chez les patients pr\u00e9sentant un bloc auriculo-ventriculaire complet :<\/strong>\u00a0<\/p><p>Le risque de traitement inappropri\u00e9 de la tachycardie supraventriculaire est inexistant, ce qui permet un r\u00e9glage relativement lent des taux d\u00e9clenchant des interventions par le dispositif, sans diminuer sa sp\u00e9cificit\u00e9. Les algorithmes de discrimination sont donc inutiles, car toutes les tachycardies spontan\u00e9es sont d'origine ventriculaire.<\/p><p><strong>Chez les patients jeunes et actifs :<\/strong><\/p><p>Le risque d'une th\u00e9rapie inappropri\u00e9e d\u00e9livr\u00e9e pour une acc\u00e9l\u00e9ration physiologique de la fr\u00e9quence cardiaque ou pour une fibrillation auriculaire doit \u00eatre pris en compte dans la d\u00e9cision de programmer plusieurs zones th\u00e9rapeutiques, tout en essayant de limiter le chevauchement entre les fr\u00e9quences d\u00e9clenchant les interventions du d\u00e9fibrillateur et les fr\u00e9quences cardiaques physiologiques.<\/p><p><strong>Chez les patients pr\u00e9sentant un syndrome de Brugada :<\/strong><\/p><p>L'apparition d'une TV organis\u00e9e est hautement improbable, alors que le risque de fibrillation auriculaire est accru. Par cons\u00e9quent, la programmation d'une zone de d\u00e9tection et de traitement de la TV en plus d'une zone de FV est probablement non seulement inutile, mais aussi risqu\u00e9e. L'ajout d'une zone suppl\u00e9mentaire de surveillance permet l'enregistrement de tachy-arythmies lentes, ce qui aide \u00e0 adapter la programmation. Une seule zone de FV est g\u00e9n\u00e9ralement programm\u00e9e avec une limite inf\u00e9rieure basse de cette zone de fr\u00e9quence relativement \u00e9lev\u00e9e (&gt;210 bpm).\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c3e220b elementor-widget elementor-widget-heading\" data-id=\"c3e220b\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Compter les cycles<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0c70b4d elementor-widget elementor-widget-text-editor\" data-id=\"0c70b4d\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>La classification des intervalles est bas\u00e9e sur la dur\u00e9e RR instantan\u00e9e et la moyenne des 4 derniers cycles.<\/p><p>Le DAI mesure les intervalles et les compte dans des cases. Un intervalle est la p\u00e9riode de temps (en millisecondes) entre un intervalle et le suivant (rythm\u00e9 et\/ou d\u00e9tect\u00e9) dans une chambre. L'intervalle actuel (CI) est l'intervalle de temps entre les \u00e9v\u00e9nements ventriculaires d\u00e9tect\u00e9s.
