La pr\u00e9valence de la fibrillation auriculaire chez les patients candidats \u00e0 la resynchronisation biventriculaire est \u00e9lev\u00e9e et augmente avec la symptomatologie : 5% pour les patients en classe I de la NYHA, 10 \u00e0 25% pour les patients en classe II et III, 50% pour les patients en classe IV. La pr\u00e9sence d\u2019une fibrillation auriculaire r\u00e9duit la probabilit\u00e9 de r\u00e9ponse apr\u00e8s resynchronisation. En effet, une capture biventriculaire compl\u00e8te et permanente, pr\u00e9 requis indispensable \u00e0 une r\u00e9ponse positive, est compromise chez les patients en fibrillation auriculaire. Un certain nombre de patients en fibrillation auriculaire pr\u00e9sentent un rythme ventriculaire rapide et irr\u00e9gulier incompatible avec une capture permanente. De plus, les patients en fibrillation auriculaire ne b\u00e9n\u00e9ficient pas d\u2019une resynchronisation auriculo-ventriculaire, ce m\u00e9canisme expliquant \u00e0 lui seul une partie de r\u00e9sultats positifs obtenus apr\u00e8s resynchronisation chez les patients en rythme sinusal.<\/p><p>Chez le patient resynchronis\u00e9, il est n\u00e9cessaire d\u2019\u00eatre relativement agressif sur le plan th\u00e9rapeutique pour maintenir le rythme sinusal le plus longtemps possible ou pour contr\u00f4ler les fr\u00e9quences.<\/p><p>Les diff\u00e9rentes options th\u00e9rapeutiques incluent:<\/p><ol><li>l\u2019introduction d\u2019une dose de charge d\u2019amiodarone et la r\u00e9alisation d\u2019un choc \u00e9lectrique pour permettre le retour en rythme sinusal ;<\/li><li>un traitement par amiodarone ou l\u2019ablation des veines pulmonaires et du substrat auriculaire gauche pour maintenir le rythme sinusal;<\/li><li>l\u2019introduction d\u2019un traitement ralentisseur pour contr\u00f4ler la fr\u00e9quence et favoriser une stimulation biventriculaire ;\u00a0<\/li><li>la programmation d\u2019algorithmes sp\u00e9cifiques qui forcent la stimulation sur activit\u00e9 ventriculaire d\u00e9tect\u00e9e ;<\/li><li>l\u2019ablation du faisceau de His pour supprimer toute comp\u00e9tition entre rythme spontan\u00e9 et rythme stimul\u00e9.<\/li><\/ol><p>\u00a0<\/p><p>L\u2019ablation par radiofr\u00e9quence de la jonction atrioventriculaire est un traitement valid\u00e9 permettant la restauration de fr\u00e9quences cardiaques adapt\u00e9es au repos comme \u00e0 l\u2019effort (coupl\u00e9e \u00e0 la programmation d\u2019un asservissement) et d\u2019une stimulation biventriculaire permanente en \u00e9liminant la comp\u00e9tition avec le rythme spontan\u00e9.<\/p><p>La programmation d'algorithmes d\u00e9clenchant une stimulation (pseudo-VT) chez les patients en fibrillation auriculaire peut \u00eatre inefficace car le rythme reste rapide et la fusion entre l'activation spontan\u00e9e et la stimulation ventriculaire est souvent incompl\u00e8te, voire absente (pseudo-fusion).\u00a0<\/p><p>L'ablation des veines pulmonaires et du substrat de l'oreillette gauche a \u00e9t\u00e9 r\u00e9alis\u00e9e, bien que les r\u00e9sultats chez les patients souffrant d'insuffisance cardiaque, dont l'oreillette gauche est dilat\u00e9e, soient sous-optimaux.<\/p><p>Il semble donc logique dans un premier temps d\u2019essayer de restaurer puis de maintenir le rythme sinusal pour pr\u00e9server la contribution atriale au remplissage et pour maintenir un r\u00e9gime de fr\u00e9quence physiologique au repos comme \u00e0 l\u2019effort. Si cette strat\u00e9gie \u00e9choue, il faut alors privil\u00e9gier le contr\u00f4le de fr\u00e9quence avec un traitement b\u00e9tabloquant adapt\u00e9 et ne pas h\u00e9siter \u00e0 proposer une ablation du faisceau de His si le r\u00e9sultat est incomplet, les patients avec conduction pr\u00e9serv\u00e9e et faible pourcentage de stimulation ne tirant pas b\u00e9n\u00e9fice de cette th\u00e9rapeutique.<\/p><p>Il est possible \u00e9galement de programmer un certain nombre d\u2019algorithmes dont l\u2019objectif est la pr\u00e9vention de la fibrillation auriculaire en se basant sur diff\u00e9rents m\u00e9canismes : augmentation du pourcentage de stimulation atriale, r\u00e9duction des pauses post-extrasytoliques, stimulation atriale non comp\u00e9titive.
