Cela signifie que 5 des 8 derniers \u00e9v\u00e9nements auriculaires doivent \u00eatre plus rapides que la fr\u00e9quence d'intervention programm\u00e9e pour que le changement de mode se produise.<\/p><p><strong>Crit\u00e8re de r\u00e9solution<\/strong><\/p><p>Le crit\u00e8re de r\u00e9solution utilise un compteur roulant X de 8 avec une valeur X par d\u00e9faut de 5. Cela signifie que 5 des 8 derniers \u00e9v\u00e9nements auriculaires doivent \u00eatre plus lents que la fr\u00e9quence d'intervention programm\u00e9e pour un retour au mode de stimulation programm\u00e9.<\/p><p><strong>Taux de base lors de la commutation de mode<\/strong><\/p><p>Il s'agit du taux de stimulation de base lorsque le changement de mode est actif. La valeur s\u00e9lectionn\u00e9e est ajout\u00e9e \u00e0 la valeur de la fr\u00e9quence de base programm\u00e9e pour devenir la fr\u00e9quence de base pendant le changement de mode. Par d\u00e9faut, cette valeur est de +10 bpm.<\/p><p><strong>Stabilisation du d\u00e9bit lors du changement de mode<\/strong><\/p><p>Cette fonction est con\u00e7ue pour minimiser les changements soudains de fr\u00e9quence dans le ventricule qui peuvent se produire en cas de fibrillation auriculaire et de conduction intacte. L'appareil d\u00e9termine une moyenne de fr\u00e9quence ventriculaire sur quatre battements et fournit une stimulation de soutien ventriculaire chaque fois que la fr\u00e9quence descend en dessous de la fr\u00e9quence moyenne moins 10 bpm.<\/p><p><strong>Protection Lock-In 2:1<\/strong><\/p><p>La gestion du verrouillage 2:1 est une extension de la fonction de changement de mode. Au cours d'un \u00e9pisode de tachycardie auriculaire ou de flutter auriculaire, si le d\u00e9lai AV et les intervalles de protection du champ lointain sont programm\u00e9s de telle sorte qu'un \u00e9v\u00e9nement auriculaire intrins\u00e8que sur deux tombe dans la fen\u00eatre de protection du champ lointain, le stimulateur cardiaque peut ne pas commuter et un suivi rapide inappropri\u00e9 jusqu'\u00e0 la fr\u00e9quence de suivi sup\u00e9rieure peut se produire. La fonction de protection 2:1 Lock-in a \u00e9t\u00e9 con\u00e7ue pour d\u00e9tecter la tachycardie et favoriser la commutation de mode.<\/p><p>La fonction de gestion du verrouillage 2:1 comprend des phases de suspicion, de confirmation et de cl\u00f4ture :<\/p><p>\u00a0<\/p><p><em>Suspicion<\/em><\/p><p>Le stimulateur cardiaque suspecte un verrouillage 2:1 lorsque les crit\u00e8res suivants sont remplis :<\/p><ul><li>le taux de r\u00e9ponse du rythme ventriculaire est sup\u00e9rieur \u00e0 100 bpm<\/li><li>8 s\u00e9quences V-pace-A-sense (VpAs) cons\u00e9cutives se sont produites avec une longueur moyenne inf\u00e9rieure au crit\u00e8re de longueur VA du verrouillage 2:1 ; ce crit\u00e8re de longueur VA est bas\u00e9 sur le d\u00e9lai AV (AsVp) et les intervalles de protection du champ lointain (FFB).<\/li><li>l'\u00e9cart moyen de ces 8 intervalles VpAs est inf\u00e9rieur au crit\u00e8re de stabilit\u00e9 du verrouillage 2:1, d\u00e9fini comme \u00e9tant de 50 ms<\/li><\/ul><p>\u00a0<\/p><p><em>Confirmation<\/em><\/p><p>Lorsque les crit\u00e8res de suspicion sont remplis, le d\u00e9lai AV est augment\u00e9 de l'intervalle de protection du champ lointain programm\u00e9 (jusqu'\u00e0 un maximum de 300 ms). Si l'\u00e9v\u00e9nement d\u00e9tect\u00e9 dans la fen\u00eatre FFP se d\u00e9place avec l'\u00e9v\u00e9nement \u00e0 rythme ventriculaire pendant l'extension du d\u00e9lai AV, il s'agit d'un \u00e9v\u00e9nement de diaphonie d\u00fb \u00e0 la stimulation ventriculaire. Toutefois, s'il ne se d\u00e9place pas avec l'\u00e9v\u00e9nement \u00e0 rythme ventriculaire lorsque le d\u00e9lai AV est prolong\u00e9, il s'agit d'un \u00e9v\u00e9nement auriculaire intrins\u00e8que (flutter auriculaire).<\/p><ul><li>si un \u00e9v\u00e9nement auriculaire est d\u00e9tect\u00e9 pendant le d\u00e9lai AV et que la fr\u00e9quence auriculaire d\u00e9tect\u00e9e est inf\u00e9rieure \u00e0 la fr\u00e9quence de d\u00e9tection de changement de mode programm\u00e9e, un verrouillage 2:1 est confirm\u00e9<\/li><li>dans le cas contraire, le verrouillage 2:1 n'est pas confirm\u00e9 et le d\u00e9lai AV diminue progressivement jusqu'\u00e0 la valeur programm\u00e9e ; la fonction de gestion du verrouillage 2:1 est suspendue pendant 120 secondes.<\/li><\/ul><p>\u00a0<\/p><p><em>Cessation d'activit\u00e9<\/em><\/p><ul><li>le stimulateur cardiaque passe imm\u00e9diatement en mode de poursuite non auriculaire lorsque les crit\u00e8res de confirmation sont remplis<\/li><li>un \u00e9v\u00e9nement de changement de mode est enregistr\u00e9 dans la m\u00e9moire<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>","protected":false},"excerpt":{"rendered":"<p>Biotronik &#8211; Atrial arrhythmia management Content Mode-Switching The device provides mode switching to change pacing modes as a result of atrial tachycardias and to avoid tracking of non-physiologic atrial rates due to paroxysmal atrial arrhythmias.
