Cela implique la programmation d'une sensibilit\u00e9 \u00e9lev\u00e9e et de p\u00e9riodes r\u00e9fractaires courtes. Le seuil de d\u00e9tection, au lieu d'\u00eatre fix\u00e9 \u00e0 une valeur fixe comme dans un stimulateur cardiaque, est automatiquement adapt\u00e9 en fonction de l'amplitude de l'onde R pr\u00e9c\u00e9dente. Le circuit de d\u00e9tection est aveugle pendant la p\u00e9riode de suppression, qui est programm\u00e9e \u00e0 une valeur courte pour \u00e9viter la d\u00e9tection multiple de la m\u00eame d\u00e9polarisation, tout en conservant la capacit\u00e9 de d\u00e9tecter une tachycardie tr\u00e8s rapide. \u00c0 la fin de la p\u00e9riode d'inhibition, l'adaptation de la sensibilit\u00e9 commence \u00e0 un pourcentage de l'amplitude de l'onde R d\u00e9tect\u00e9e, avant de diminuer progressivement jusqu'\u00e0 la valeur programm\u00e9e la plus basse. Chaque fabricant a choisi des valeurs sp\u00e9cifiques pour a) la sensibilit\u00e9 de base \u00e0 partir du moment o\u00f9 l'onde R a \u00e9t\u00e9 d\u00e9tect\u00e9e, et b) le taux d'augmentation de la sensibilit\u00e9 pendant le cycle. La sensibilit\u00e9 minimale nominale est de 0,8 mV et la p\u00e9riode de suppression (p\u00e9riode r\u00e9fractaire ventriculaire absolue de d\u00e9tection post-ventriculaire) est de 110 ms dans les d\u00e9fibrillateurs les plus r\u00e9cents et de 80 ms dans les mod\u00e8les plus anciens. Ces valeurs permettent une d\u00e9tection correcte de la FV chez la majorit\u00e9 des patients lorsque l'amplitude de l'onde R en rythme sinusal est &gt;5 mV. Depuis le mod\u00e8le 740, la bande passante a \u00e9t\u00e9 \u00e9largie pour se situer entre 24 et 100 Hz.\u00a0<\/p><p><strong><a>Programmation standard<\/a><\/strong><\/p><p>L'analyse d'une EGM ventriculaire comprend une composante positive et une composante n\u00e9gative. Les d\u00e9fibrillateurs Biotronik mesurent l'amplitude du pic positif ou n\u00e9gatif le plus \u00e9lev\u00e9. L'apparition d'une onde R est suivie d'une p\u00e9riode de suppression de 110 ms, apr\u00e8s laquelle la d\u00e9tection ventriculaire commence pour des amplitudes &gt;50% de l'onde R d\u00e9tect\u00e9e (seuil sup\u00e9rieur \u00e0 50%). Ce seuil persiste pendant 350 ms (360 ms pour les d\u00e9fibrillateurs plus anciens que le mod\u00e8le 740) apr\u00e8s la d\u00e9tection de l'onde R (240 ms apr\u00e8s la fin de la p\u00e9riode d'occultation). Ensuite, la sensibilit\u00e9 augmente jusqu'\u00e0 un seuil de 25% (seuil inf\u00e9rieur de 25%) pendant une dur\u00e9e de 156 ms. Dans les d\u00e9fibrillateurs Lumax 740, la sensibilit\u00e9 augmente r\u00e9guli\u00e8rement tout au long du cycle cardiaque par paliers d\u00e9cr\u00e9mentiels de -12,5% toutes les 156 ms jusqu'\u00e0 ce qu'elle atteigne soit la sensibilit\u00e9 nominale de 0,8 mV, soit une valeur programmable entre 0,5 et 2,5 mV. Pour ce faire, le seuil sup\u00e9rieur est maintenu \u00e0 50% de l'amplitude de l'onde R pendant 110 ms. Dans les anciens mod\u00e8les d'appareils, la p\u00e9riode de suppression durait 80 ms, de sorte que le seuil sup\u00e9rieur \u00e9tait maintenu \u00e0 50% pendant 30 ms, tandis que le seuil inf\u00e9rieur \u00e9tait \u00e0 25% de l'amplitude de l'onde R. Dans les mod\u00e8les plus r\u00e9cents, la d\u00e9tection se fait par l'interm\u00e9diaire d'un d\u00e9tecteur de mouvement, d'un capteur de pression et d'un capteur de temp\u00e9rature. Dans les mod\u00e8les plus r\u00e9cents, la d\u00e9tection commence directement \u00e0 25% de l'onde R, \u00e0 la fin de la p\u00e9riode de suppression de 110 ms. Le filtre passe-haut est \u00e9galement augment\u00e9 \u00e0 32 Hz pour \u00e9viter une surd\u00e9tection de l'onde T. Ce r\u00e9glage ne doit pas \u00eatre utilis\u00e9 si l'appareil est \u00e9quip\u00e9 d'un syst\u00e8me d'alarme.
