Il y a jusqu'\u00e0 5 tentatives (sauf pour SafeR et Dplus)*.<\/li><\/ul><p>\u00a0<\/p><p><em>Phase d'attente<\/em><\/p><p>8 cycles cardiaques sont d\u00e9livr\u00e9s \u00e0 la fr\u00e9quence de stimulation actuelle.<\/p><ul><li>amplitude ventriculaire programm\u00e9e \u00e0 5 V (largeur d'impulsion programm\u00e9e)<\/li><li>la phase d'\u00e9talonnage peut commencer si l'appareil peut stimuler avec un d\u00e9lai AV &gt; 125 ms et une fr\u00e9quence de stimulation &lt; 95 min-1, pendant 8 cycles cardiaques<\/li><\/ul><p>\u00a0<\/p><p><em>Phase de calibration<\/em><\/p><p>La phase d'\u00e9talonnage est n\u00e9cessaire pour d\u00e9montrer une diff\u00e9renciation appropri\u00e9e de la r\u00e9ponse au potentiel \u00e9voqu\u00e9 (EPR) et de la polarisation post-stimulation de l'\u00e9lectrode. Le signal est analys\u00e9 dans une fen\u00eatre de 65 ms apr\u00e8s la stimulation ventriculaire.<\/p><ul><li>capture : polarisation post-stimulation+ r\u00e9ponse \u00e9voqu\u00e9e<\/li><li>non capture : polarisation post-pacing<\/li><\/ul><p>R\u00e9duire le risque de battements de fusion pendant la phase d'\u00e9talonnage et le test de seuil :<\/p><ul><li>le d\u00e9lai AV est r\u00e9duit de 65 ms en DDD ou VDD<\/li><li>l'intervalle d'\u00e9chappement est raccourci de 65 ms en mode VVI, DDI et commutation en mode de repli<\/li><\/ul><p>Pendant la phase d'\u00e9talonnage, diff\u00e9rentes amplitudes de stimulation sont d\u00e9livr\u00e9es :<\/p><ul><li>3 stimuli ventriculaires \u00e0 4 V (largeur d'impulsion programm\u00e9e) : premier stimulus pour \u00e9viter l'effet Wedensky ; calibration sur les 2 spikes suivants avec \u00e9valuation du signal dans une fen\u00eatre de 65 ms ; moyenne des 2 mesures<\/li><li>3 stimuli ventriculaires \u00e0 2 V + impulsion de s\u00e9curit\u00e9 \u00e0 2,5 V\/1 ms : premier stimulus pour \u00e9viter l'effet Wedensky ; calibration sur les 2 spikes suivants avec \u00e9valuation du signal dans une fen\u00eatre de 65 ms ; moyenne des 2 mesures<\/li><li>comparaison des mesures moyennes \u00e0 4V et 2V pour d\u00e9montrer la similitude de la r\u00e9ponse \u00e9voqu\u00e9e<\/li><li>Pendant 2 cycles, l'appareil \u00e9value la possibilit\u00e9 de fusion. \u00c0 la fin du DAV, il \u00e9value le signal dans une fen\u00eatre de 65 ms et mesure l'amplitude ventriculaire. Ensuite, un pic de sauvegarde de 2,5 V\/1 ms est appliqu\u00e9 \u00e0 la fin de la fen\u00eatre de 65 ms. Pendant la fen\u00eatre de 65 ms<ul><li>Si l'appareil identifie une d\u00e9polarisation ventriculaire spontan\u00e9e similaire \u00e0 une \u201ccapture\u201d ventriculaire, des battements de fusion peuvent \u00eatre suspect\u00e9s pour la v\u00e9rification de la capture 4 V et 2 V (ou il peut \u00e9galement s'agir d'un battement ectopique). Cela signifie que la fusion peut \u00e9galement se produire pendant le test de seuil. L'amplitude des \u00e9v\u00e9nements auriculaires et ventriculaires est surveill\u00e9e en permanence avec un ajustement automatique des sensibilit\u00e9s cycle par cycle. L'amplitude du PAC n'est pas incluse dans la moyenne. Elle est remplac\u00e9e par la valeur 0,4 mV pour le calcul de l'amplitude moyenne.
