ventriculaire suivant une stimulation atriale (patients avec conduction auriculoventriculaire pr\u00e9serv\u00e9e)<br \/>7) Le dispositif mesure p\u00e9riodiquement le seuil de stimulation atriale tous les jours \u00e0 1 heure du matin<br \/>8) L\u2019amplitude de stimulation atriale est adapt\u00e9e automatiquement en ajoutant 1 V \u00e0 la valeur du seuil mesur\u00e9 dans les limites d\u2019une valeur minimale programmable<br \/>9) La dur\u00e9e d\u2019impulsion est programmable <br \/>10) Il n\u2019y a pas de v\u00e9rification cycle-\u00e0-cycle de l\u2019efficacit\u00e9 de la stimulation ventriculaire \u00e0 l\u2019exception du mode SafeR.<\/p><ul><li>\u00a0Autoseuil sur auto : mesures r\u00e9guli\u00e8res du seuil de stimulation ventriculaire + adaptation de la programmation<\/li><li>\u00a0Mesure du seuil r\u00e9alis\u00e9e syst\u00e9matiquement toutes les 6 heures (bas\u00e9e sur l\u2019analyse de la r\u00e9ponse \u00e9voqu\u00e9e)<\/li><li>4 mesures de seuil par jour avec adaptation pour les 6 heures suivantes (pas de v\u00e9rification cycle \u00e0 cycle)<\/li><li>\u00a0Adaptation automatique de l\u2019amplitude au double de la valeur de seuil (marge de s\u00e9curit\u00e9 de 100%) dans la limite d\u2019une valeur minimale programmable (1.5, 2, 2.5, 3, 3.5 ou 4 V)<\/li><li>\u00a0Autoseuil sur suivi : mesures r\u00e9guli\u00e8res du seuil de pratique<\/strong> <br \/>Le test de capture est bas\u00e9 sur la diff\u00e9rentiation entre le potentiel \u00e9voqu\u00e9 (EPR) et la polarisation r\u00e9siduelle de la sonde (Polar). l\u2019objectif de ces stimulations de s\u00e9curit\u00e9 est d\u2019assurer une capture ventriculaire en cas de seuil de stimulation sup\u00e9rieur \u00e0 2 V<\/li><li>\u00a0Si ces deux s\u00e9ries donnent des mesures acceptables et comparables, le stimulateur applique l\u2019\u00e9tape suivante<\/li><li>Deux \u201cstimulations \u00e0 0 V\u201d sont suivies par une p\u00e9riode de mesure de la r\u00e9ponse \u00e9voqu\u00e9e et de la polarisation ; elles permettent de faire le diagnostic de fusion qui peut alt\u00e9rer la mesure du seuil ; en effet si apr\u00e8s une\u201cstimulation \u00e0 0 V\u201d, des potentiels \u00e9voqu\u00e9s spontan\u00e9s sont mesur\u00e9s, ils sont r\u00e9v\u00e9lateurs de la pr\u00e9sence d\u2019une fusion possible ou de d\u00e9polarisations ectopiques lors des mesures pr\u00e9c\u00e9dentes (\u00e0 2 et 4 V) ; Le fonctionnement est bas\u00e9 <br \/>sur 3 grands principes :<\/p><ul><li>Le seuil de stimulation atriale est r\u00e9alis\u00e9 une fois par jour, la nuit (1h du matin)<\/li><li>Il existe 2 m\u00e9thodes pour mesurer le seuil de stimulation (PR ou AR) choisies automatiquement <br \/>par le dispositif en fonction de l\u2019existence d\u2019un rythme sinusal stable et sup\u00e9rieur \u00e0 50 bpm (seuil r\u00e9alis\u00e9 avec incr\u00e9ment de l\u2019amplitude de stimulation) ou d\u2019une conduction atrio-ventriculaire en 1\/1 avec intervalles AR stables (seuil r\u00e9alis\u00e9 avec d\u00e9cr\u00e9ment de l\u2019amplitude de stimulation)<\/li><li>\u00a0L\u2019absence de capture atriale est \u00e9valu\u00e9e de fa\u00e7on indirecte et est sugg\u00e9r\u00e9e devant la reprise d\u2019une activit\u00e9 atriale spontan\u00e9e<\/li><li>L\u2019amplitude de stimulation atriale est automatiquement adapt\u00e9e pour les 24 heures suivantes \u00e0 une valeur sup\u00e9rieure de 1 V par rapport au seuil mesur\u00e9 ou \u00e0 la valeur minimale de stimulation atriale programm\u00e9e (valeur la plus \u00e9lev\u00e9e entre les 2)<\/li><\/ul><p><\/p><h2><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-efb4f9b elementor-widget elementor-widget-heading\" data-id=\"efb4f9b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Sensing<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3359536 elementor-widget elementor-widget-text-editor\" data-id=\"3359536\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><\/p><p><\/p><p><strong>Autosensing<\/strong><br \/>Il est possible de programmer la fonction Autosensing Atrial et Ventriculaire avec adaptation automatique des sensibilit\u00e9s A et V aux variations d\u2019amplitude des \u00e9v\u00e8nements d\u00e9tect\u00e9s. L\u2019amplitude des \u00e9v\u00e8nements atriaux et ventriculaires est monitor\u00e9e en <br \/>permanence avec adaptation automatique cycle \u00e0 cycle des sensibilit\u00e9s.
