(pr\u00e9f\u00e9rence \u00e0 la stimulation atriale), Medtronic (Pr\u00e9f\u00e9rence de stimulation atriale), Abbott (AF suppression) et Microport CRM-Sorin (Overdrive du rythme sinusal).<\/li>\n<li>Il existe un algorithme permettant d'\u00e9viter la pause post extrasystolique atriale et\/ou augmentant la fr\u00e9quence de stimulation \u00e0 la suite d'une extrasystole atriale pour les stimulateurs Boston Scientific (PSA\/ProACt), Medtronic (Stabilisation du rythme atrial) et Microport CRM-Sorin (Suppression de pause sur ESA et Acc\u00e9l\u00e9ration fr\u00e9quence sur ESA).<\/li>\n<li>Il existe un algorithme permettant de r\u00e9gulariser la fr\u00e9quence ventriculaire lors d'un passage en FA (\u00e9viter les diastoles longues) pour les stimulateurs Biotronik (stabilisation de fr\u00e9quence), Boston Scientific (R\u00e9gulation de la Fr\u00e9quence Ventriculaire) et Medtronic (R\u00e9ponse \u00e0 une FA conduite).<\/li>\n<li>Il existe un algorithme d'acc\u00e9l\u00e9ration de la fr\u00e9quence de stimulation en fin d'\u00e9pisode d\u2019arythmie sur les stimulateurs Medtronic (Stimulation rapide post-commutation de plus rapides que la fr\u00e9quence atriale intrins\u00e8que (overdrive).<\/p><p>Lorsque l\u2019algorithme d\u00e9tecte deux ondes P dans une fen\u00eatre de 16 cycles, l\u2019appareil augmente la fr\u00e9quence de stimulation pour neutraliser la conduction spontan\u00e9e. Apr\u00e8s stimulation \u00e0 la fr\u00e9quence de l\u2019Algorithme AF Suppression pendant le nombre de cycles indiqu\u00e9s par le param\u00e8tre Nombre de cycles d\u2019overdrive (15 \u00e0 40 ; nominal 15), l\u2019appareil ralentit sa fr\u00e9quence jusqu\u2019\u00e0 ce qu\u2019il d\u00e9tecte deux nouvelles ondes P. Si deux ondes P sont d\u00e9tect\u00e9es, il repasse en stimulation rapide (overdrive). S\u2019il ne d\u00e9tecte pas deux ondes P, la stimulation reprend selon le r\u00e9glage de la fr\u00e9quence de base, de la fr\u00e9quence de repos ou de la fr\u00e9quence capteur.<\/p><p>Calcul de la fr\u00e9quence d\u2019overdrive: pour des fr\u00e9quences de simulation 151 bpm, overdrive fixe de 5 bpm (= fr\u00e9quence maximale d\u2019overdrive); pour des fr\u00e9quences situ\u00e9es entre 60 et 150 bpm, l\u2019algorithme AF Suppression augmente la fr\u00e9quence de stimulation d\u2019un delta compris entre la fr\u00e9quence minimale d\u2019overdrive et la fr\u00e9quence maximale d\u2019overdrive.<\/p><p>Retour \u00e0 la fr\u00e9quence de base: ce param\u00e8tre non programmable d\u00e9termine l\u2019intervalle et la pente de r\u00e9duction depuis la fr\u00e9quence d\u2019overdrive jusqu\u2019\u00e0 la fr\u00e9quence de base, fr\u00e9quence de repos ou fr\u00e9quence indiqu\u00e9e par le capteur; si la fr\u00e9quence d\u2019overdrive est sup\u00e9rieure \u00e0 100 bpm, 8ms sont ajout\u00e9es \u00e0 chaque cycle jusqu\u2019\u00e0 100 bpm puis + 12ms jusqu\u2019\u00e0 la fr\u00e9quence de base.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div spontan\u00e9e en augmentant la fr\u00e9quence de stimulation atriale.<\/p><p>\u00a0<\/p><h6>Stabilisation de fr\u00e9quence<\/h6><p>Il s\u2019agit d\u2019un algorithme ayant pour objectif de r\u00e9duire les variations de fr\u00e9quence (alternance cycles courts \u2013 cycles longs) lors d\u2019un passage en FA quand la conduction atrio-ventriculaire est pr\u00e9serv\u00e9e. La fr\u00e9quence de stimulation atriale est donc augment\u00e9e lorsque des \u00e9v\u00e8nements atriaux d\u00e9tect\u00e9s se produisent (raccourcissement de l\u2019intervalle VA de 8 ms). La fr\u00e9quence de stimulation PSA est limit\u00e9e par la Fr\u00e9quence de stimulation maximale PSA qui est programmable. La fr\u00e9quence PSA\/ProAct indiqu\u00e9e est limit\u00e9e par la valeur de la fr\u00e9quence de stimulation maximale PSA\/ProAct.<\/p><h6>R\u00e9gulation de la Fr\u00e9quence Ventriculaire (RFV)<\/h6><p>Chez la majorit\u00e9 des patients l\u2019arythmie auriculaire engendre une r\u00e9ponse ventriculaire rapide et irr\u00e9guli\u00e8re, l\u2019irr\u00e9gularit\u00e9 des cycles ventriculaires pouvant alt\u00e9rer l\u2019h\u00e9modynamique du patient.<\/p><p>L'objectif de cet algorithme est de r\u00e9duire la variabilit\u00e9 des cycles RR en augmentant le pourcentage de stimulation. La stimulation atriale non comp\u00e9titive (SANC) est con\u00e7ue pour \u00e9viter le d\u00e9clenchement d\u2019une arythmie atriale par une stimulation au cours de la p\u00e9riode r\u00e9fractaire relative de l\u2019oreillette.<\/p><p>Lorsque la SANC est programm\u00e9e sur Marche, un \u00e9v\u00e9nement atrial r\u00e9fractaire d\u00e9tect\u00e9 au cours de la PRAPV d\u00e9clenche une p\u00e9riode SANC programmable pendant laquelle aucune stimulation atriale ne peut se produire.<\/p><p>Lorsque la stimulation atriale est retard\u00e9e par le fonctionnement de la SANC, le stimulateur tente de conserver une fr\u00e9quence ventriculaire stable en r\u00e9duisant le d\u00e9lai AV stimul\u00e9 qui suit. Apr\u00e8s chaque \u00e9v\u00e9nement d\u00e9tect\u00e9 atrial non r\u00e9fractaire, le dispositif diminue l\u2019intervalle de stimulation atriale de la valeur du d\u00e9cr\u00e9ment d\u2019intervalle programm\u00e9e. Les impulsions de stimulation atriale d\u00e9livr\u00e9es pour la PSA sont annot\u00e9es PP (stimulation proactive) sur le marqueur d\u2019\u00e9v\u00e9nements.<\/p><p>Si l'\u00e9v\u00e9nement atrial suivant est un autre \u00e9v\u00e9nement d\u00e9tect\u00e9 non r\u00e9fractaire, l'intervalle de stimulation est \u00e0 nouveau d\u00e9cr\u00e9ment\u00e9. Cette progression continue jusqu'\u00e0 ce que la fr\u00e9quence de stimulation d\u00e9passe la fr\u00e9quence spontan\u00e9e, r\u00e9sultant en un rythme atrial stimul\u00e9. Cependant, la valeur de la fr\u00e9quence maximale programm\u00e9e fournit une limite de fr\u00e9quence pour la pr\u00e9f\u00e9rence de stimulation atriale. Apr\u00e8s une p\u00e9riode programmable de stimulation atriale \u00e0 100%, le stimulateur d\u00e9cro\u00eet progressivement la fr\u00e9quence de stimulation \u00e0 la recherche du battement spontan\u00e9 suivant. Le battement sinusal suivant relance la pr\u00e9f\u00e9rence de stimulation.<\/p><h6>Stabilisation du rythme auriculaire<\/h6><p>La stabilisation du rythme atrial (SRA) est une fonction programmable con\u00e7ue pour \u00e9viter la pause sinusale qui suit une ESA (s\u00e9quences cycles court-long-court qui peuvent entra\u00eener le d\u00e9marrage brutal de certaines tachyarythmies atriales). Elle r\u00e9agit \u00e0 une ESA en \u00e9levant instantan\u00e9ment la fr\u00e9quence de stimulation atriale puis en ralentissant r\u00e9guli\u00e8rement la fr\u00e9quence jusqu\u2019\u00e0 la fr\u00e9quence spontan\u00e9e, la fr\u00e9quence de stimulation programm\u00e9e ou la fr\u00e9quence capteur.<\/p><p>Lorsque le dispositif est activ\u00e9 par une ESA, il d\u00e9livre une impulsion de stimulation \u00e0 l\u2019intervalle pr\u00e9matur\u00e9 augment\u00e9 d\u2019un pourcentage de cet intervalle (d\u00e9fini par le param\u00e8tre Incr\u00e9ment intervalles en pourcentage programm\u00e9).
