Ceci implique donc la programmation d\u2019une sensibilit\u00e9 \u00e9lev\u00e9e et de p\u00e9riodes r\u00e9fractaires courtes. Le seuil de d\u00e9tection n\u2019est pas programm\u00e9 \u00e0 une valeur fixe comme dans un stimulateur cardiaque mais s\u2019adapte automatiquement par rapport \u00e0 l\u2019amplitude de l\u2019onde R pr\u00e9c\u00e9dente. La sensibilit\u00e9 augmente ensuite au cours du cycle, afin de rechercher un \u00e9ventuel signal de faible amplitude.<\/p><p>Le circuit de d\u00e9tection est aveugle pendant la p\u00e9riode r\u00e9fractaire post-d\u00e9tection ventriculaire qui est programm\u00e9e courte pour \u00e9viter de d\u00e9tecter plusieurs fois une m\u00eame d\u00e9polarisation tout en ayant la possibilit\u00e9 de d\u00e9tecter une tachycardie tr\u00e8s rapide. Cette p\u00e9riode r\u00e9fractaire est absolue pour certains constructeurs (blanking) ou relative pour d'autres (possibilit\u00e9 de rechercher l'amplitude maximale du signal mais sans possibilit\u00e9 de compter 2 fois le m\u00eame signal). A la fin de cette p\u00e9riode r\u00e9fractaire, le niveau d\u2019adaptation de la sensibilit\u00e9 d\u00e9bute \u00e0 un pourcentage de l\u2019amplitude de l\u2019onde R d\u00e9tect\u00e9e puis diminue progressivement jusqu\u2019\u00e0 atteindre la valeur minimale programm\u00e9e. Les d\u00e9fibrillateurs modernes permettent la programmation de plusieurs zones de d\u00e9tection d\u2019arythmies bas\u00e9es sur les intervalles RR d\u00e9tect\u00e9s. Chaque zone peut b\u00e9n\u00e9ficier d'une programmation sp\u00e9cifique pour la discrimination et les th\u00e9rapies. Le nombre de zones et la gamme des fr\u00e9quences programm\u00e9es doivent \u00eatre adapt\u00e9s en fonction des caract\u00e9ristiques, des ant\u00e9c\u00e9dents et de l'indication d'implantation du patient.<\/p><p><em>En pr\u00e9vention secondaire :<\/em> chez un patient en insuffisance cardiaque, la programmation des zones de fr\u00e9quence est adapt\u00e9e en fonction du cycle de la TV motivant l\u2019implantation. En g\u00e9n\u00e9ral, la borne basse de la zone de TV est programm\u00e9e 20 battements par minute plus lente que la TV clinique, un traitement anti arythmique d\u00e9but\u00e9 ou renforc\u00e9 apr\u00e8s l'implantation \u00e9tant susceptible de diminuer la fr\u00e9quence de la TV.<\/p><p><em>Pr\u00e9vention primaire :<\/em> chez l\u2019insuffisant cardiaque, le risque de pr\u00e9senter une tachycardie ventriculaire dans les suites de l\u2019implantation est cons\u00e9quent et justifie probablement la programmation de 2 zones de d\u00e9tection et de traitement de tachycardie ventriculaire en plus de la zone de fibrillation ventriculaire. L\u2019int\u00e9r\u00eat principal de programmer 2 zones distinctes de tachycardie ventriculaire est d\u2019avoir la possibilit\u00e9 de traiter sp\u00e9cifiquement les tachycardies ventriculaires tr\u00e8s rapides en privil\u00e9giant la stimulation anti-tachycardique indolore par rapport au choc \u00e9lectrique. Ceci \u00e0 \u00e9t\u00e9 d\u00e9montr\u00e9 dans les \u00e9tudes cliniques PAIN FREE I et PAIN FREE II : la stimulation anti-tachycardique permet d\u2019\u00e9viter 3 chocs sur 4 dans la zone de TV rapide sans augmenter significativement les \u00e9v\u00e8nements ind\u00e9sirables. Ceci conduit \u00e0 un b\u00e9n\u00e9fice significatif en termes de qualit\u00e9 de vie.<\/p><p><em>Chez un patient pr\u00e9sentant un bloc auriculo-ventriculaire complet :<\/em> le risque de th\u00e9rapie inappropri\u00e9e sur une tachycardie supra-ventriculaire conduite est nulle ce qui permet de r\u00e9gler des fr\u00e9quences d\u2019intervention de l\u2019appareil relativement basses sans alt\u00e9rer la sp\u00e9cificit\u00e9. Les algorithmes de discrimination sont inutiles puisque toute tachycardie spontan\u00e9e est d\u2019origine ventriculaire.<\/p><p><em>Chez les patients jeunes et actifs :<\/em> le risque de traitement inappropri\u00e9 sur une acc\u00e9l\u00e9ration physiologique ou sur une fibrillation auriculaire conduite doit \u00eatre int\u00e9gr\u00e9 dans la d\u00e9cision de programmer plusieurs zones d\u2019intervention, en essayant de limiter le chevauchement entre fr\u00e9quences d\u2019intervention du d\u00e9fibrillateur et fr\u00e9quences physiologiques du patient.<\/p><p><em>Chez un patient atteint du syndrome de Brugada :<\/em> la survenue d'une tachycardie ventriculaire organis\u00e9e est extr\u00eamement rare alors que le risque de fibrillation auriculaire est accru. La programmation d\u2019une zone de d\u00e9tection et de traitement des tachycardies ventriculaires en plus de la zone FV est donc probablement inutile voire dangereuse. La programmation d\u2019une zone suppl\u00e9mentaire de monitorage seul permet d\u2019enregistrer d\u2019\u00e9ventuelles arythmies pour des fr\u00e9quences plus basses et ainsi d\u2019adapter la programmation.
