Il faut adopter la modalit\u00e9 de traitement la moins agressive et la moins douloureuse pour mettre fin \u00e0 une arythmie. Le principe de la stimulation anti-tachycardique est de capturer l'arythmie et d'interrompre une TV organis\u00e9e en p\u00e9n\u00e9trant dans son circuit de propagation \u00e0 travers les ventricules. Le ventricule doit donc \u00eatre stimul\u00e9 \u00e0 un rythme plus rapide que celui de la tachycardie. La stimulation anti-tachycardique est indolore, diminue la consommation d'\u00e9nergie et \u00e9pargne les batteries. Elle doit donc \u00eatre privil\u00e9gi\u00e9e comme traitement de premi\u00e8re intention des troubles du rythme ventriculaire organis\u00e9s, m\u00eame lorsqu'ils sont rapides. L'efficacit\u00e9 de ce type de th\u00e9rapies a \u00e9t\u00e9 confirm\u00e9e dans une large gamme de taux de TV puisque la stimulation anti-tachycardique s'est constamment r\u00e9v\u00e9l\u00e9e efficace pour mettre fin \u00e0 90% des tachycardies ventriculaires lentes (&lt; 200 bpm) avec cependant un faible risque d&#039;acc\u00e9l\u00e9ration (1%-5%). Ces observations ont permis de repositionner le DAI comme un dispositif d&#039;ATP avec une d\u00e9fibrillation de secours uniquement en cas de besoin.<\/p><p style=\"color: #000000;\">Diff\u00e9rents facteurs \u00e9lectrophysiologiques influencent directement la capacit\u00e9 d'une s\u00e9quence de stimulation anti-tachycardique \u00e0 r\u00e9duire une TV : la dur\u00e9e du cycle tachycardique, la pr\u00e9sence d'une lacune excitable dans le circuit de la TV et le temps de conduction entre le site de stimulation et le site de formation de l'impulsion.<\/p><p style=\"color: #000000;\">La probabilit\u00e9 d'efficacit\u00e9 d'une s\u00e9quence ATP est \u00e9galement influenc\u00e9e par des facteurs sp\u00e9cifiques au sch\u00e9ma de stimulation lui-m\u00eame : le moment du stimulus, l'intervalle de couplage, le taux et le nombre d'impulsions dans la s\u00e9quence, la proximit\u00e9 du site de stimulation par rapport au circuit et le nombre de sites de stimulation.<\/p><p style=\"color: #000000;\">Plusieurs r\u00e9glages doivent \u00eatre programm\u00e9s pour optimiser l'efficacit\u00e9 de ce type de th\u00e9rapie \u00e9valu\u00e9e comme le rapport entre la fin de l'\u00e9pisode et l'effet ind\u00e9sirable (acc\u00e9l\u00e9ration de l'arythmie) :<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b32bb9b elementor-widget elementor-widget-text-editor\" data-id=\"b32bb9b\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><strong>Type de s\u00e9quences : rafale, rampe<\/strong><\/p><p>Une s\u00e9quence de stimulation en rafale consiste en un train d'impulsions de stimulation avec un intervalle interstimulus \u00e9gal. Il s'agit du type de s\u00e9quence le plus couramment utilis\u00e9 dans la pratique clinique.<\/p><p>Une s\u00e9quence de stimulation en rampe consiste en un train d'impulsions de stimulation avec un intervalle interstimulus automatiquement d\u00e9croissant.<\/p><p>Dans cette gamme de TV (&lt; 200 bpm), la stimulation en rafale et la stimulation en rampe se sont av\u00e9r\u00e9es d&#039;une efficacit\u00e9 \u00e9quivalente pour mettre fin \u00e0 la TV avec un risque faible similaire de provoquer une acc\u00e9l\u00e9ration.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-82f4aa5 elementor-widget elementor-widget-text-editor\" data-id=\"82f4aa5\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><strong>Nombre de s\u00e9quences de stimulation<\/strong><\/p><p>Le nombre de s\u00e9quences programm\u00e9es varie en fonction de la vitesse de la tachycardie. Dans une zone de TV lente, un grand nombre de s\u00e9quences peut \u00eatre programm\u00e9 afin de retarder au maximum la d\u00e9livrance du choc pour une tachycardie qui ne menace pas la survie \u00e0 court terme du patient. Il est en effet possible de ne pas programmer de choc \u00e9lectrique pour cette zone de tachycardie lente (VT-1). Pour les tachycardies entre 150 et 200 bpm, il est habituel de programmer 3 \u00e0 6 s\u00e9quences cons\u00e9cutives d'ATP.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-496952e elementor-widget elementor-widget-text-editor\" data-id=\"496952e\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><strong>Nombre d'impulsions par s\u00e9quence<\/strong><\/p><p>En moyenne, 5 \u00e0 15 impulsions cons\u00e9cutives sont programm\u00e9es dans chaque salve. Si le nombre d'impulsions est insuffisant, elles risquent de ne pas p\u00e9n\u00e9trer dans le circuit de la tachycardie et de rester inefficaces. En revanche, si le nombre d'impulsions est excessif, elles risquent de se terminer et de provoquer \u00e0 nouveau la tachycardie. Un stimulus de stimulation suppl\u00e9mentaire peut \u00eatre ajout\u00e9 syst\u00e9matiquement d'une s\u00e9quence \u00e0 l'autre.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-2fd11ac elementor-widget elementor-widget-text-editor\" data-id=\"2fd11ac\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><strong>Couplage par rapport au cycle de stimulation<\/strong><\/p><p>Le cycle de couplage (d\u00e9lai entre le dernier \u00e9v\u00e9nement ventriculaire d\u00e9tect\u00e9 et le premier stimulus) et le cycle de stimulation (d\u00e9lai entre le premier et le deuxi\u00e8me stimulus) peuvent \u00eatre programm\u00e9s diff\u00e9remment selon les entreprises. Plus l'intervalle de couplage est court, plus la th\u00e9rapie est agressive et plus le risque d'acc\u00e9l\u00e9ration de la tachycardie est \u00e9lev\u00e9. Les deux intervalles de couplage peuvent \u00eatre programm\u00e9s en pourcentage ou en cycles fixes (en ms). Il semble plus judicieux de programmer en pourcentages afin de les adapter au rythme de la tachycardie et \u00e0 ses variations. Le pourcentage est calcul\u00e9 \u00e0 partir de la moyenne des 4 cycles pr\u00e9c\u00e9dant la th\u00e9rapie.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-4446a63 elementor-widget elementor-widget-text-editor\" data-id=\"4446a63\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><strong>Couplage minimum<\/strong><\/p><p>Il est possible de programmer une limite de cadence au-del\u00e0 de laquelle, quelle que soit la programmation, l'appareil ne sera pas cadenc\u00e9. Si, au cours d'une rampe, par exemple, l'intervalle minimum a \u00e9t\u00e9 atteint, les cycles suivants seront rythm\u00e9s \u00e0 cette longueur de cycle de couplage minimum sans autre d\u00e9cr\u00e9mentation.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-2a357ef elementor-widget elementor-widget-text-editor\" data-id=\"2a357ef\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><strong>Dur\u00e9e la plus longue de l'ATP<\/strong><\/p><p>Cette dur\u00e9e (d\u00e9nomination diff\u00e9rente selon les entreprises) est le temps apr\u00e8s lequel les s\u00e9quences d'ATP sont interrompues au profit de la d\u00e9livrance d'un choc \u00e9lectrique. Cette caract\u00e9ristique a \u00e9t\u00e9 con\u00e7ue pour \u00e9viter un retard dans la d\u00e9livrance du premier choc par la d\u00e9livrance d'un trop grand nombre de s\u00e9quences d'ATP.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-74c85e7 elementor-widget elementor-widget-text-editor\" data-id=\"74c85e7\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><strong>Amplitude de l'impulsion de stimulation<\/strong><\/p><p>Les chances d'une capture ventriculaire fiable peuvent \u00eatre augment\u00e9es en programmant ind\u00e9pendamment l'amplitude et la dur\u00e9e des impulsions ATP.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-2a4ff24 elementor-widget elementor-widget-text-editor\" data-id=\"2a4ff24\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><strong>Site de stimulation de l'ATP<\/strong><\/p><p>Le changement du site de stimulation peut modifier la capacit\u00e9 d'une impulsion de stimulation \u00e0 interagir avec le circuit de tachycardie et \u00e0 y mettre fin. Dans les DAI \u00e0 une ou deux d\u00e9rivations, la seule possibilit\u00e9 est la stimulation du ventricule gauche. En revanche, les dispositifs de TRC offrent la possibilit\u00e9 d'un site de stimulation alternatif. L'ATP d\u00e9livr\u00e9e \u00e0 partir de la sonde ventriculaire gauche ou des sondes de stimulation BiV peut, d'un point de vue th\u00e9orique, augmenter le succ\u00e8s par rapport \u00e0 la stimulation \u00e0 partir de la sonde ventriculaire droite, \u00e9tant donn\u00e9 que de nombreux circuits r\u00e9entrants apparaissent dans les parois du ventricule gauche.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>","protected":false},"excerpt":{"rendered":"<p>ATP in VT zone A priority of defibrillator programming is to minimize the delivery of shocks without compromising the patient\u2019s safety.
