Dans un d\u00e9fibrillateur, le boitier positionn\u00e9 dans la loge \u00e0 proximit\u00e9 des muscles pectoraux ne faisant pas partie du circuit de d\u00e9tection, les myopotentiels pectoraux ne devraient donc pas g\u00e9n\u00e9rer de surd\u00e9tection. L'amplitude de ces myopotentiels est plus importante quand enregistr\u00e9s au niveau du canal de choc qui int\u00e8gre le boitier (d\u00e9tection entre le co\u00efl ventriculaire droit et le boitier). La surd\u00e9tection sur ce canal ne modifie pas traditionnellement le compte des arythmies mais peut alt\u00e9rer la qualit\u00e9 d'une discrimination bas\u00e9e sur l'analyse de la morphologie des signaux mesur\u00e9s sur le canal de choc. Elle peut \u00e9galement alt\u00e9rer le fonctionnement de nouveaux algorithmes bas\u00e9s sur la comparaison entre d\u00e9tection sur le canal de d\u00e9tection et d\u00e9tection sur le canal de choc SecureSense, algorithme de discrimination du bruit).<\/p><p>En revanche, si il existe un d\u00e9faut d'isolant (typiquement une \u00e9rosion g\u00e9n\u00e9rant une fuite de courant) au niveau de la portion de sonde situ\u00e9e dans la loge (frottement entre le boitier et la sonde), le canal de d\u00e9tection peut alors surd\u00e9tecter les myopotentiels pectoraux ce qui peut entrainer la survenue d'une inhibition de la stimulation et\/ou la survenue de th\u00e9rapies inappropri\u00e9es.<\/p><p>L'analyse des EGMs retrouve dans ce cadre, la pr\u00e9sence de signaux tr\u00e8s rapides (haute fr\u00e9quence) non physiologiques. La surd\u00e9tection peut \u00eatre reproduite par des mouvements isom\u00e9triques du bras homolat\u00e9ral au boitier ou par une manipulation de la sonde dans la poche.\n\nDevant une suspicion de surd\u00e9tection de myopotentiels pectoraux, une radiographie pulmonaire doit \u00eatre r\u00e9alis\u00e9e avec un contr\u00f4le complet du dispositif (valeurs d'imp\u00e9dance, seuils de stimulation et de d\u00e9tection). L'existence d'une valeur d'imp\u00e9dance (stimulation et\/ou d\u00e9fibrillation) anormalement basse ou d'une baisse brutale de cette valeur est en faveur d'une rupture d'isolant.<\/p><p>Dans de tr\u00e8s rares cas, une surd\u00e9tection de myopotentiels peut \u00eatre observ\u00e9e lorsqu'un syst\u00e8me DF1 a \u00e9t\u00e9 implant\u00e9 avec inversion des pinoches dans le connecteur.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>","protected":false},"excerpt":{"rendered":"<p>Pectoral myopotentials Oversensing of pectoral myopotentials was relatively common in patients implanted with an unipolar pacemaker and could result in inhibition of ventricular pacing and syncope in pacemaker dependent patients.
