L'intef\u00e9rence peut survenir par conduction si le patient est en contact direct avec la source \u00e9mettrice ou par radiation si le patient est situ\u00e9 \u00e0 l'int\u00e9rieur d'un champ \u00e9lectromagn\u00e9tique. Les d\u00e9fibrillateurs les plus r\u00e9cents sont prot\u00e9g\u00e9s contre la grande majorit\u00e9 des sources d'interf\u00e9rence que le patient peut rencontrer dans sa vie de tous les jours. Les signaux parasites sont typiquement filtr\u00e9s, l'analyse \u00e9tant restreinte \u00e0 une bande passante \u00e9troite correspondant aux signaux physiologiques (filtres passe-haut et passe-bas). Cependant, la sensibilit\u00e9 \u00e9lev\u00e9e et adaptative requise dans un d\u00e9fibrillateur pour la d\u00e9tection correcte des signaux lors d'une fibrillation ventriculaire, peut favoriser la d\u00e9tection de signaux non physiologiques correspondant \u00e0 la m\u00eame bande passante. Les signaux correspondant \u00e0 une interf\u00e9rence \u00e9lectromagn\u00e9tique peuvent ne pas \u00eatre filtr\u00e9s de fa\u00e7on appropri\u00e9e et conduire \u00e0 des cons\u00e9quences plus ou moins s\u00e9v\u00e8res allant de la survenue de th\u00e9rapies inappropri\u00e9es, \u00e0 une inhibition chez un patient d\u00e9pendant, un repli inappropri\u00e9 pour faux de diagnostic d'arythmie supraventriculaire, une stimulation ventriculaire rapide synchronis\u00e9e sur une surd\u00e9tection atriale, une suspension de la d\u00e9tection des th\u00e9rapies ou un repli vers un mode asynchrone. De fa\u00e7on exceptionnelle, une interf\u00e9rence avec un champ de haute intensit\u00e9 peut causer une l\u00e9sion permanente des circuits.<\/p><p>Le diagnostic d'interf\u00e9rence \u00e9lectromagn\u00e9tique est bas\u00e9 sur la concordance entre une exposition \u00e0 une source au moment de l'\u00e9pisode et une surd\u00e9tection de signaux caract\u00e9ristiques (rapides, r\u00e9guliers et saturant la ligne de base). Une interf\u00e9rence \u00e9lectromagn\u00e9tique \u00e0 la fr\u00e9quence \"secteur\" (60 Hz aux Etats Unis et 50 Hz pour l'Europe) survient quand le patient est en contact physique avec un \u00e9quipement \u00e9lectrique mal isol\u00e9. Si la surd\u00e9tection est prolong\u00e9e, un choc \u00e9lectrique unique permet g\u00e9n\u00e9ralement l'interruption de la surd\u00e9tection puisque le patient interrompt g\u00e9n\u00e9ralement imm\u00e9diatement son activit\u00e9. L'interf\u00e9rence \u00e9lectromagn\u00e9tique est plus fr\u00e9quente pour une sonde bipolaire int\u00e9gr\u00e9e que pour une \"vraie\" d\u00e9tection bipolaire, l'antenne de d\u00e9tection \u00e9tant plus large. Les signaux caract\u00e9ristiques sont \u00e0 haute fr\u00e9quence et d'\u00e9vidence non physiologiques, sont d\u00e9tect\u00e9s sur les diff\u00e9rents canaux disponibles (possible diagnostic de bitachycardie, FA + FV) et sont g\u00e9n\u00e9ralement de plus grande amplitude sur le canal de choc que sur le canal de d\u00e9tection. L'aspect sinuso\u00efdal caract\u00e9ristique d'un signal 50 ou 60 Hz peut \u00eatre mis en \u00e9vidence en modifiant la vitesse de d\u00e9filement (16 \u00e0 20 ms entre les signaux).<\/p><p>La mesure pr\u00e9ventive principale consiste \u00e0 identifier la source \u00e9mettrice et \u00e9viter d'utiliser certains instruments mal isol\u00e9s. Diff\u00e9rentes compagnies (Boston Scientific, Microport CRM-Sorin, et Abbott) ont incorpor\u00e9 un algorithme de r\u00e9jection du bruit bas\u00e9 sur les caract\u00e9ristiques de ces signaux.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-c1ed3e3 e-con-full e-flex e-con e-parent\" data-id=\"c1ed3e3\" data-element_type=\"container\" data-settings=\"{&quot;background_background&quot;:&quot;classic&quot;}\">\n\t\t\t\t<div class=\"elementor-element elementor-element-2bc38f9 elementor-widget elementor-widget-text-editor\" data-id=\"2bc38f9\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><strong>Utilisation du bistouri \u00e9lectrique et fonctionnement \u00e0 l'aimant<\/strong><\/p><p>Quand un patient implant\u00e9 d'un d\u00e9fibrillateur subit une intervention chirurgicale, il doit \u00eatre conseill\u00e9 au chirurgien d'utiliser le bistouri \u00e9lectrique en mode bipolaire, pendant des dur\u00e9es courtes et de placer les plaques \u00e0 distance du boitier de fa\u00e7on \u00e0 garder le syst\u00e8me de d\u00e9fibrillation en dehors du champ g\u00e9n\u00e9r\u00e9 par le bistouri \u00e9lectrique. Le d\u00e9fibrillateur doit \u00eatre temporairement d\u00e9sactiv\u00e9 pour \u00e9liminer compl\u00e8tement le risque d'interf\u00e9rence, le patient devant \u00eatre monitor\u00e9, avec un d\u00e9fibrillateur externe \u00e0 proximit\u00e9 disponible si n\u00e9cessaire. Deux options peuvent \u00eatre privil\u00e9gi\u00e9es pour d\u00e9sactiver le dispositif: application d'un aimant sur la loge ou d\u00e9programmation du dispositif. L'application d'un aimant est g\u00e9n\u00e9ralement pr\u00e9f\u00e9r\u00e9e car 1) en cas de survenue d'une vraie arythmie compromettant la survie du patient, l'\u00e9quipe m\u00e9dicale peut imm\u00e9diatement r\u00e9activer le syst\u00e8me en retirant l'aimant (reprise de la d\u00e9tection des tachycardies imm\u00e9diatement au retrait de l'aimant) 2) l'absence ou le retard dans la r\u00e9activation des th\u00e9rapies ne sont pas rares quand le d\u00e9fibrillateur a \u00e9t\u00e9 d\u00e9programm\u00e9.<\/p><p>Il existe certaines sp\u00e9cificit\u00e9s du fonctionnement \u00e0 l'aimant en fonction des compagnies: pas d'effet sur la stimulation sauf pour les dispositifs Sorin, interruption de l'effet \u00e0 l'aimant apr\u00e8s 8 heures d'application sur les dispositifs Biotronik, r\u00e9ponse \u00e0 l'aimant programmable sur les dispositifs Saint Jude M\u00e9dical et Boston Scientific (marche versus arr\u00eat).
