Diff\u00e9rentes \u00e9tudes ont montr\u00e9 cependant qu'une proportion importante des arythmies ventriculaires rapides (dur\u00e9e de cycle &lt; 320 ms) et class\u00e9es en zone de FV sont r\u00e9guli\u00e8res et monomorphes (TVs rapides potentiellement traitables par stimulation anti-tachycardique). Des doutes sur le manque d&#039;efficacit\u00e9 et sur le risque d&#039;acc\u00e9l\u00e9ration en une arythmie extr\u00eamement rapide syncopale gr\u00e9vant le pronostic du patient ont historiquement limit\u00e9 l&#039;utilisation de s\u00e9quences de stimulation anti-tachycardique pour cette gamme de tachycardies. M\u00eame si l&#039;efficacit\u00e9 d&#039;un choc \u00e9lectrique pour r\u00e9duire une arythmie ventriculaire rapide est non d\u00e9battue, un choc est douloureux et augmente sensiblement la consommation \u00e9nerg\u00e9tique. La succession de plusieurs chocs pour un m\u00eame patient est donc associ\u00e9e avec un risque d&#039;usure pr\u00e9matur\u00e9e des batteries et une alt\u00e9ration sensible de la qualit\u00e9 de vie des patients (nombreux cas d\u00e9crits de d\u00e9pression ou d&#039;anxi\u00e9t\u00e9 induites par une s\u00e9rie de chocs).<\/p><p style=\"color: #000000;\">L'\u00e9tude PainFree Rx (\u00e9tude prospective, randomis\u00e9e, multicentrique) a montr\u00e9 qu'une seule s\u00e9quence de stimulation anti-tachycardique (8 battements \u00e0 88%) permettait de r\u00e9duire une proportion importante de tachycardies rapides en zone de FV et permettait un b\u00e9n\u00e9fice significatif en termes de qualit\u00e9 de vie, en r\u00e9duisant le nombre de chocs d\u00e9livr\u00e9s sans augmenter le risque de mort subite, de syncope ou d'acc\u00e9l\u00e9ration de la tachycardie.<\/p><p style=\"color: #000000;\">D\u00e9livrer une s\u00e9quence de stimulation anti-tachycardique en zone de FV parait donc sans danger, efficace, indolore, permet de r\u00e9duire l'usure des batteries et d'am\u00e9liorer la qualit\u00e9 de vie et devrait donc \u00eatre propos\u00e9 en premi\u00e8re intention pour cette gamme de tachycardies (&lt; 250 battements\/minute)..<\/p><p style=\"color: #000000;\">Les constructeurs ont d\u00e9velopp\u00e9 diff\u00e9rents algorithmes avec un certain nombre de diff\u00e9rences pour d\u00e9livrer un nombre limit\u00e9 de s\u00e9quences de stimulation pendant ou avant la charge.\n\nL\u2019ATP pendant la charge permet un traitement indolore des tachycardies ventriculaires sans risquer de retarder le traitement par choc quand cela est n\u00e9cessaire. Une fois le diagnostic de la FV \u00e9tabli, la charge d\u00e9bute et un ATP est d\u00e9livr\u00e9 simultan\u00e9ment. Si la FV est reconfirm\u00e9e apr\u00e8s l\u2019ATP, le choc est d\u00e9livr\u00e9. Si le patient est revenu en rythme sinusal, le choc n\u2019est pas d\u00e9livr\u00e9. Cela permet donc de traiter de fa\u00e7on indolore la tachycardie si l\u2019ATP est efficace, sans retarder la survenue du choc si l\u2019ATP est inefficace. En revanche, m\u00eame si l\u2019ATP est efficace, il n\u2019y a pratiquement pas d\u2019\u00e9conomie d\u2019\u00e9nergie.\u00a0<\/p><p style=\"color: #000000;\">L\u2019ATP avant la charge permet de r\u00e9duire la consommation d\u2019\u00e9nergie. Une fois le diagnostic de la FV \u00e9tabli, l\u2019ATP est d\u00e9livr\u00e9. Si la FV est reconfirm\u00e9e apr\u00e8s la survenue de l\u2019ATP, la charge du condensateur commence. Si l\u2019arythmie est r\u00e9duite, la charge n\u2019est pas initi\u00e9e. Cela permet donc d\u2019\u00e9conomiser une charge de condensateurs si l\u2019ATP est efficace.
