font-size: 16px;\"><li><p>v\u00e9rifier l'int\u00e9grit\u00e9 du syst\u00e8me haute tension<\/p><\/li><li><p>v\u00e9rifier la d\u00e9tection et la re-d\u00e9tection correctes des \u00e9lectrogrammes intracardiaques pendant la fibrillation ventriculaire<\/p><\/li><li><p>v\u00e9rifier que la marge de s\u00e9curit\u00e9 de la d\u00e9fibrillation est suffisamment large<\/p><\/li><\/ul><p style=\"color: #000000;\">Malgr\u00e9 les risques encourus, ce test \u00e9tait syst\u00e9matiquement effectu\u00e9 avec des dispositifs plus anciens, implant\u00e9s pour des indications de pr\u00e9vention secondaire, chez des patients pr\u00e9sentant un risque \u00e9lev\u00e9 de mort cardiaque subite r\u00e9currente. Les chocs \u00e9taient la seule th\u00e9rapie disponible et les \u00e9checs de d\u00e9fibrillation \u00e9taient relativement fr\u00e9quents. De nos jours, les \u00e9checs de d\u00e9fibrillation sont rares, la majorit\u00e9 des patients sont implant\u00e9s dans le cadre de la pr\u00e9vention primaire et une grande partie des TV rapides peuvent \u00eatre interrompues par l'ATP. En outre, la puissance maximale des chocs d\u00e9livr\u00e9s par les derniers d\u00e9fibrillateurs implantables est plus \u00e9lev\u00e9e et associ\u00e9e \u00e0 une plus grande marge de s\u00e9curit\u00e9.<\/p><p>Cela a incit\u00e9 de nombreux centres d'implantation \u00e0 reconsid\u00e9rer la n\u00e9cessit\u00e9 d'un test syst\u00e9matique \u00e0 la fin de la proc\u00e9dure et \u00e0 proc\u00e9der plut\u00f4t au cas par cas.\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ab334ce elementor-widget elementor-widget-text-editor\" data-id=\"ab334ce\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<h6>Induction de la fibrillation ventriculaire<\/h6><p>Plusieurs moyens d'induction de la FV sont disponibles dans la pratique clinique :<\/p><ol><li style=\"font-size: 16px;\"><em>D\u00e9livrance d'ondes de choc T<\/em>La stimulation ventriculaire : un choc de faible \u00e9nergie est d\u00e9livr\u00e9 pendant la p\u00e9riode de vuln\u00e9rabilit\u00e9 ventriculaire afin de provoquer une arythmie ventriculaire (le plus souvent une FV). Une s\u00e9quence de stimulation ventriculaire, \u00e0 un rythme fixe, est suivie par la d\u00e9livrance, \u00e0 travers les \u00e9lectrodes, d'un choc \u00e9lectrique de faible \u00e9nergie pendant la p\u00e9riode de vuln\u00e9rabilit\u00e9 ventriculaire. La d\u00e9livrance du choc n\u00e9cessite la charge pr\u00e9alable des condensateurs.\u00a0<\/li><li style=\"font-size: 16px;\"><em>Fourniture de courant alternatif<\/em>La stimulation : une salve de stimulation rapide (courant alternatif) est d\u00e9livr\u00e9e pour induire une arythmie (TV ou FV). Plusieurs rythmes sont disponibles selon les fabricants : 33 HZ \u00e0 30 ms entre les impulsions ; 50 HZ \u00e0 20 ms ; 20 HZ \u00e0 50 ms.\u00a0<\/li><li style=\"font-size: 16px;\"><em>Fourniture d'impulsions \u00e0 haut d\u00e9bit et \u00e0 haut rendement<\/em>Les impulsions sont d\u00e9livr\u00e9es par l'\u00e9lectrode de choc avec 2 longueurs de cycle diff\u00e9rentes pour induire une arythmie ventriculaire (le plus souvent une FV).\u00a0<\/li><li style=\"font-size: 16px;\">Le taux de r\u00e9ussite, c'est-\u00e0-dire le rapport induction VF \/ tentative, semble plus \u00e9lev\u00e9 avec l'administration de courant continu ou alternatif (peut minimiser le nombre de tentatives n\u00e9cessaires) qu'avec l'administration de chocs sur l'onde T. En outre, la simplicit\u00e9 de programmation est plus grande avec le courant continu ou alternatif puisqu'un seul param\u00e8tre programmable est n\u00e9cessaire. Si la proc\u00e9dure est r\u00e9alis\u00e9e sous anesth\u00e9sie locale, le courant alternatif indolore est pr\u00e9f\u00e9r\u00e9 aux autres m\u00e9thodes d'induction.<\/li><\/ol>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-1768fab elementor-widget elementor-widget-text-editor\" data-id=\"1768fab\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<h6 style=\"color: #000000;\">D\u00e9livrance du choc \u00e9lectrique<\/h6><p style=\"color: #000000;\">Au cours de la proc\u00e9dure d'induction, le choc \u00e9lectrique peut \u00eatre d\u00e9livr\u00e9 par trois m\u00e9thodes diff\u00e9rentes :<\/p><ol style=\"color: #000000; font-size: 16px;\"><li>Automatique : l'appareil d\u00e9livre le traitement apr\u00e8s confirmation de l'\u00e9pisode dans la zone FV.\u00a0<\/li><li>Retard\u00e9 : l'appareil d\u00e9livre le traitement apr\u00e8s confirmation de l'\u00e9pisode dans la zone FV et apr\u00e8s un d\u00e9lai programmable.\u00a0<\/li><li>Manuel : l'appareil charge les condensateurs et l'op\u00e9rateur choisit le moment de l'administration du traitement. Si la proc\u00e9dure est effectu\u00e9e sous anesth\u00e9sie locale, la FV est d\u00e9clench\u00e9e par le courant alternatif et le choc est d\u00e9livr\u00e9 manuellement apr\u00e8s que le patient a perdu connaissance.<\/li><\/ol>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-eb551c7 elementor-widget elementor-widget-text-editor\" data-id=\"eb551c7\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<h6 style=\"color: #000000;\">Tests de d\u00e9fibrillation en pratique clinique<\/h6><p style=\"color: #000000;\">Un certain nombre de protocoles diff\u00e9rents peuvent \u00eatre utilis\u00e9s. Nombre d'entre eux ne visent pas \u00e0 mesurer le seuil de d\u00e9fibrillation r\u00e9el, mais \u00e0 d\u00e9terminer qu'un appareil est susceptible de fournir une marge de s\u00e9curit\u00e9 ad\u00e9quate. En effet, la d\u00e9termination d'une courbe compl\u00e8te probabilit\u00e9 de succ\u00e8s\/\u00e9nergie d\u00e9livr\u00e9e n\u00e9cessiterait beaucoup plus d'\u00e9pisodes de fibrillation\/d\u00e9fibrillation que ce qui est acceptable en pratique et en toute s\u00e9curit\u00e9. Dans la pratique clinique, diff\u00e9rents protocoles peuvent \u00eatre appliqu\u00e9s :<\/p><ol style=\"color: #000000; font-size: 16px;\"><li><em>Essais par paliers jusqu'\u00e0 l'\u00e9chec<\/em>L'\u00e9nergie du premier choc est g\u00e9n\u00e9ralement programm\u00e9e \u00e0 10 joules de moins que la puissance maximale de l'appareil. Des \u00e9nergies progressivement plus faibles sont ensuite utilis\u00e9es lors d'inductions successives (par exemple des \u00e9tapes de 5J d'\u00e9nergies plus faibles pour chaque test successif) jusqu'\u00e0 ce qu'il y ait un \u00e9chec de la d\u00e9fibrillation. Le seuil de d\u00e9fibrillation est consid\u00e9r\u00e9 comme l'\u00e9nergie la plus faible ayant r\u00e9ussi. Ce type de test est rarement utilis\u00e9 en clinique en raison du nombre excessivement \u00e9lev\u00e9 de chocs qu'il n\u00e9cessite. De plus, le mot seuil n'est pas ad\u00e9quat pour la d\u00e9fibrillation en raison de la nature probabiliste de la d\u00e9fibrillation. Cette m\u00e9thode peut conduire \u00e0 programmer le premier choc \u00e0 une valeur inf\u00e9rieure \u00e0 la sortie maximale.\u00a0<\/li><li><em>Essai de r\u00e9sistance \u00e0 un choc unique \u00e0 deux reprises<\/em>L'amplitude d'un premier choc est g\u00e9n\u00e9ralement inf\u00e9rieure de 10 J \u00e0 la capacit\u00e9 maximale de l'appareil. Pour v\u00e9rifier l'efficacit\u00e9 du choc, la m\u00eame amplitude est test\u00e9e une seconde fois. Si les 2 chocs r\u00e9duisent l'arythmie, cela sugg\u00e8re une marge de s\u00e9curit\u00e9 appropri\u00e9e. Cette m\u00e9thode peut conduire \u00e0 programmer le premier choc \u00e0 la capacit\u00e9 maximale de l'appareil.\u00a0<\/li><li><em>Test de r\u00e9sistance \u00e0 un choc unique sur<\/em>ce : un seul choc (par exemple 14 joules : au moins 15 joules en dessous de la puissance maximale de l'appareil) est test\u00e9. De m\u00eame, si ce choc est r\u00e9ussi, il sugg\u00e8re une marge de s\u00e9curit\u00e9 appropri\u00e9e.<\/li><\/ol><p style=\"color: #000000;\">Au moins 3 \u00e0 5 minutes doivent s'\u00e9couler entre chaque induction de FV afin de permettre une r\u00e9cup\u00e9ration h\u00e9modynamique et de minimiser l'effet cumulatif des chocs.\u00a0<span style=\"font-size: 1rem;\">Si le choc d\u00e9livr\u00e9 par le d\u00e9fibrillateur implantable est inefficace, \u22651 choc(s) de sauvetage peut \u00eatre d\u00e9livr\u00e9 par un dispositif externe ou par le syst\u00e8me implant\u00e9, \u00e0 l'\u00e9nergie programmable la plus \u00e9lev\u00e9e. Le choc interne ne comporte pas de risque de l\u00e9sion thermique de la peau, mais pr\u00e9sente l'inconv\u00e9nient d'\u00eatre retard\u00e9 par la re-d\u00e9tection de la FV (sauf en cas d'utilisation du mode choc command\u00e9) et par le nouveau temps de charge n\u00e9cessaire pour atteindre l'\u00e9nergie maximale.\u00a0<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0baf775 elementor-widget elementor-widget-heading\" data-id=\"0baf775\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Informations sp\u00e9cifiques \u00e0 l'entreprise<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-4549885 elementor-widget elementor-widget-heading\" data-id=\"4549885\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h4 class=\"elementor-heading-title elementor-size-default\">Abbott<\/h4>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e08bc51 elementor-widget elementor-widget-text-editor\" data-id=\"e08bc51\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Il existe trois m\u00e9thodes pour induire une FV : l'onde de choc T, la stimulation rapide et le DC Fibber.<sup>TM<\/sup><\/p><p><strong>1. Il est possible de programmer une puissance variable : Low Fib V d\u00e9livre une forme d'onde de stimulation de 9 volts (dur\u00e9e d'impulsion de 1,1 ms, intervalle de 50 ms entre les stimuli (20 Hz)) ; High VF d\u00e9livre une forme d'onde de stimulation de 15 volts (dur\u00e9e d'impulsion de 1,1 ms, intervalle de 50 ms entre les stimuli (20 Hz)). Le train d'induction est d\u00e9livr\u00e9 tant que l'op\u00e9rateur appuie sur le bouton et que la liaison t\u00e9l\u00e9m\u00e9trique est maintenue (ne peut exc\u00e9der 15 secondes).
