La pr\u00e9-excitation sera visible si l\u2019on ralentit la conduction nodale par des man\u0153uvres vagales ou de l\u2019adenosine, ou si l\u2019on stimule l\u2019oreillette proche de la voie accessoire. Les diff\u00e9rentes classifications propos\u00e9es sont bas\u00e9es sur l\u2019analyse de la polarit\u00e9 de l\u2019onde delta et\/ou du complexe QRS. La localisation pr\u00e9cise des voies accessoires est surtout utile dans l\u2019optique d\u2019une ablation pour anticiper la voie d\u2019abord, le mat\u00e9riel n\u00e9cessaire (gaines et type de gaine, acc\u00e8s f\u00e9moral veineux ou art\u00e9riel, veineux sous clavier ou jugulaire, trans-septal) et les complications potentielles (bloc auriculo-ventriculaire notamment).\r\n\r\nL\u2019algorithme de localisation de la voie accessoire pr\u00e9sent\u00e9 ici est bas\u00e9 sur l\u2019analyse de la polarit\u00e9 de l\u2019onde delta, de la polarit\u00e9 du complexe QRS et sur la polarit\u00e9 suppos\u00e9e de l\u2019onde delta et du QRS lors de la pr\u00e9-excitation maximale (quand la d\u00e9polarisation ventriculaire se fait de fa\u00e7on pr\u00e9dominante voire exclusive via la voie accessoire, lors d\u2019une stimulation atriale rapide proche de la voie accessoire en cours d\u2019exploration \u00e9lectrophysiologique par exemple). La localisation pr\u00e9cise de la voie accessoire d\u00e9pend aussi de la position du coeur dans le thorax et de l\u2019existence d\u2019une cardiopathie sous jacente.\r\n\r\nEn pratique, la recherche de la localisation d\u2019une voie accessoire d\u00e9bute par l\u2019\u00e9tude des d\u00e9rivations V1 et V2. En l\u2019absence de cardiopathie, si l\u2019onde delta est positive en V1 (aspect de retard droit), il s\u2019agit d\u2019une pr\u00e9-excitation gauche. Si l\u2019onde delta est n\u00e9gative en V1 et positive en V2, il s\u2019agit d\u2019une pr\u00e9-excitation septale (droite ou gauche selon le site d\u2019ablation). Si la pr\u00e9-excitation est n\u00e9gative en V1 et V2 (aspect de retard gauche), alors la voie accessoire est situ\u00e9e \u00e0 droite. Selon la transition de polarit\u00e9 de l\u2019onde delta et du QRS dans les d\u00e9rivations pr\u00e9cordiales, il sera possible de d\u00e9finir si la voie accessoire est septale ou plus lat\u00e9rale. Une transition en V1-V2 (onde delta et QRS n\u00e9gatifs en VI mais positifs en V2) \u00e9voque une voie accessoire septale. Si la transition est plus tardive (V3-V4) alors il s\u2019agit d\u2019une voie accessoire lat\u00e9rale droite.\r\n\r\nEnsuite, l\u2019\u00e9tude des d\u00e9rivations inf\u00e9rieures (DII, DIII et aVF) permet de savoir si la voie accessoire est ant\u00e9rieure ou post\u00e9rieure. Si l\u2019onde delta est positive en inf\u00e9rieure, la pr\u00e9-excitation est ant\u00e9rieure car l\u2019activation ventriculaire va vers les d\u00e9rivations inf\u00e9rieures. Par contre, si l\u2019onde delta est n\u00e9gative en inf\u00e9rieure, alors la voie accessoire est post\u00e9rieure.\r\n\r\nEnfin, l\u2019\u00e9tude des d\u00e9rivations DI et aVL permet de d\u00e9finir si la voie accessoire est lat\u00e9rale gauche. Si l\u2019onde delta est n\u00e9gative en DI et aVL, il s\u2019agit d\u2019une voie lat\u00e9rale gauche car l\u2019activation ventriculaire fuit DI et aVL.
