Ces conditions anatomiques et h\u00e9modynamiques expliquent les caract\u00e9ristiques \u00e9lectriques visualis\u00e9es sur l\u2019\u00e9lectrocardiogramme \u00e0 la naissance : 1) la pression ventriculaire droite est \u00e9lev\u00e9e et \u00e9gale la pression syst\u00e9mique. On observe donc une surcharge systolique ventriculaire droite et une surcharge fr\u00e9quente de l\u2019oreillette droite; 2) la pression ventriculaire gauche peut \u00eatre plus basse que la pression ventriculaire droite si le septum ventriculaire est intact et les r\u00e9sistances art\u00e9rielles pulmonaires sont basses; il n\u2019y a donc pratiquement jamais de surcharge systolique gauche.\r\n\r\nChez les patients avec transposition des gros vaisseaux et communication interventriculaire,\r\non peut noter certaines sp\u00e9cificit\u00e9s sur l\u2019\u00e9lectrocardiogramme : l\u2019hypertrophie ventriculaire droite est g\u00e9n\u00e9ralement moins nette, avec des complexes R ou Rs observ\u00e9s dans les pr\u00e9cordiales droites et une n\u00e9gativit\u00e9 pr\u00e9dominante dans les d\u00e9rivations gauches (complexe rS) ; on peut parfois observer une hypertrophie ventriculaire gauche associ\u00e9e suspect\u00e9e sur la pr\u00e9sence d\u2019ondes q profondes ou d\u2019ondes T n\u00e9gatives dans les d\u00e9rivations pr\u00e9cordiales gauches ;\r\n\r\nLe second trac\u00e9 montre un exemple de flutter atrial commun chez le nourrisson. Il n\u2019est pas rare d\u2019observer une fr\u00e9quence atriale tr\u00e8s rapide (entre 350 et 400 bpm) avec possibilit\u00e9 de conduction atrio-ventriculaire rapide favorisant la survenue d\u2019une d\u00e9compensation cardiaque.
