Le fonctionnement, les avantages et les inconv\u00e9nients propres \u00e0 chaque mode de stimulation doivent \u00eatre compris par le m\u00e9decin assurant le suivi afin d\u2019optimiser la programmation par rapport aux sp\u00e9cificit\u00e9s des diff\u00e9rents patients implant\u00e9s. Une approche combin\u00e9e entre la soci\u00e9t\u00e9 nord-am\u00e9ricaine et la soci\u00e9t\u00e9 anglaise de stimulation et \u00e9lectrophysiologie (NASPE et BPEG) a permis de d\u00e9finir un code international (NBG) de classification des diff\u00e9rents modes de stimulation. Les divers modes de stimulation sont qualifi\u00e9s par un code \u00e0 4 lettres permettant de comprendre leur fonctionnement de base. La premi\u00e8re lettre d\u00e9finit le(s) site(s) de stimulation : ventricule (V), oreillette (A), les deux (D), simple chambre (S) ou aucune (0). La deuxi\u00e8me lettre d\u00e9finit le(s) site(s) de d\u00e9tection : m\u00eames lettres. La troisi\u00e8me lettre indique le mode de fonctionnement : inhib\u00e9 (I), d\u00e9clench\u00e9 (T, pour triggered), les deux (D), aucun des pr\u00e9c\u00e9dents (0). La quatri\u00e8me lettre indique l\u2019asservissement de fr\u00e9quence (R) ou son absence (0). \r\n\r\nSur ce premier trac\u00e9, le stimulateur a \u00e9t\u00e9 programm\u00e9 en mode 0D0. La sp\u00e9cificit\u00e9 de ce mode de \u201cstimulation\u201d est que la d\u00e9tection dans les 2 cavit\u00e9s est effective mais qu\u2019aucune stimulation (atriale ou ventriculaire) n\u2019est possible. Il est donc \u00e9videmment \u00e0 proscrire chez les patients d\u00e9pendants. Ce mode de stimulation permet donc une analyse du rythme spontan\u00e9 du patient avec visualisation concomitante de l\u2019\u00e9lectrocardiogramme et des marqueurs d\u2019\u00e9v\u00e8nements. Il s\u2019agit d\u2019un mode de test de sensibilit\u00e9. Le mode 0D0 peut \u00eatre programm\u00e9 de fa\u00e7on temporaire chez les patients non d\u00e9pendants porteurs d\u2019un pacemaker IRM-compatible devant b\u00e9n\u00e9ficier d\u2019une IRM. Le mode 0D0 de ces stimulateurs IRM compatible permet d\u2019\u00e9viter l\u2018\u00e9ventuelle r\u00e9version d\u2019un mode 0D0 traditionnel vers un mode VVI. Il ne faut \u00e9videmment pas oublier de contr\u00f4ler et reprogrammer le stimulateur apr\u00e8s l\u2019examen (en fonction des constructeurs).\r\n\r\nDans les trac\u00e9s suivants, diff\u00e9rents marqueurs sont utilis\u00e9s:\r\n\r\nAS (atrial sensing): d\u00e9tection atriale;
